STUDENT SMARTRIDER
Students attending secondary schools or colleges are entitled to concession travel on Transperth services. To obtain the concession entitlement, students must produce a student travel permit when using the services. To obtain the student travel permit, students need to apply to their school or college who issue the permit on Transperth’s behalf.
HOW TO APPLY
Hampton Senior High School is combining both the SmarRider and Library Card. One will be issued free to all students in year 8 and 11 as well as students enrolling throughout the year. Replacement cards will incur a $5.00 replacement fee.
CONDITIONS OF APPLICATION
The Student SmartRider is issued subject to the conditionsof the Public Transport Authority Act 2003 and the Public TransportAuthority Regulations 2003 and conditions of travel determined byTransperth from time to time. A Student SmartRider may only be used by the student to whom it isissued. A Student SmartRider is not transferable.It is the applicant’s responsibility to ensure that they meet the eligibilityrequirements for the issue of a Student SmartRider.
The information on this form will be recorded by the Public TransportAuthority and used to verify the student’s ongoing eligibility for the Student SmartRider with the nominated school or college.

The information provided on this application form is for theuse by the Public Transport Authority and its service providers only, andwill only be used for the purposes of conducting business under thePublic Transport Authority Act 2003.The information will not be passed onto any other personor organisation unless so directed by Law.
Please complete the following in ink and block letters (all fields mandatory) and return to the school library, where a photograph will be taken and SmartRider/Library Card ordered. Cards may then be picked up by the student approximately one week later.
