Student Can and Will Succeed Contract

Dear Mr. Polish;

I, ______, agree that I will do everything in my power to be a successful student in Mr. Polish’s Independent Studies classes at C.H.A.M.P.S. Charter School. In order for me to achieve this goal, I agree to the following:

  1. I will follow all of Mr. Polish’s rules and policies
  2. I will be respectful in Mr. Polish’s classes
  3. I will not cheat
  4. I will not use my cell phone in class
  5. I will go to Mr. Polish’s class on time every day
  6. I will go to Mr. Polish’s class with a good attitude about learning and about myself
  7. I will go to Mr. Polish’s class with the necessary materials in order for me to excel (books, paper, notebooks, pens, etc.)
  8. I will complete all of Mr. Polish’s assignments and hand them in on time
  9. I will not be ashamed to seek help from Mr. Polish if things get difficult

Literature Permission Slip

Mr. Polish will be assigning several books this semester that contain adult content. First, students will be reading Perks of Being a Wallflower, a young adult book that contains foul language, drug use, and sexual content. After that, students will be readingThe Girl on the Train, which features some profanity, violence, and mild sexual situations. Later in the semester, students will be reading The Girl with the Dragon Tattoo, a mystery that contains a rape, violence, and foul language. The final book that students read this semester will be The Kite Runner, a book about Afghanistan that also contains a rape and violence. Students must have permission to read thesebooks. Without permission, they will have to do alternate assignments instead.

If your child is in History of Film, Mr. Polish will show several R-Rated films including Psycho (1960), The Godfather, El Norte, and Do the Right Thing. Psycho is rated R for horror violence; The Godfather is rated R for brief sexual content, violence, and profanity; El Norte is rated R for some violence and profanity; and Do the Right Thing is rated R for brief sexual content, constant strong language, and some violence.
