Archdiocesan Student Accident Insurance

  • Pertains to any student in any school activity on site or off site
  • Excess Insurance -- only addresses the amount over and above personal insurance or is primary only if no personal insurance
  • Forms are available on the website: front page,

  • Claim form:
  • MOST important to fill it out correctly.
  • Don’t use old forms – Maksin is current company
  • May change companies from year-to-year to get best deal
  • Page 2 of form explains what is needed to file claim
  • (i.e., complete bill with medical codes – not a statement)
  • To help— School can maintain copies of all forms/documentsparents send in.

  • Schools Sports covered -- YES
  • Preschoolerscovered – YES
  • Out of state travel covered -- YES
  • Need special insurance for travel outside the country
  • For Travel Accident policy notify Mike Witka

Incident Report Forms

  • For Injuries to students or staff resulting in medical care

Send these to Mike Witka and keep copy for your records

  • File Incident Report on website
  • Adult employee’s injuries may need Workers’ Compensation forms from Mike Witka

Other Insurance Concerns

Teachers and after school programs

  • Covered if under their contract or work agreement, and/or if they are paid for work through the school
  • If acting in after school program as an independent contractor, they are not covered(i.e., if they are paid directly by parents). Contractor needs to provide their own proof of liabilityinsurance (contact Mike Witka for sources of insurance) (Another example: Summer basketball camp run by coach who collects money directly from parents and it does not run it through school business books -- is not covered)
  • The safest and best way is to pay for after-school/summer activities through the school
  • Title I employees who are paid through public school system are covered for extra activities if the funds for these run through the Catholic school books. If not – not covered

Volunteer workers/nurses

  • Covered under archdiocesan professional liabilitypolicies
  • Nurses are covered (volunteer or paid)

Requests for Certificate of Archdiocesan Insurance—call Mike Witka, or request online

  • Need to request Certificate of Insurance from other groups using your facility.
  • Festival rides—need Certificate of Insurance for each vendor.
