Please complete this form for any household which you have evidence that the family meet at least 2 of the 6 headline eligibility criteria for the Troubled Families Programme as detailed below.

Please provide commentary as to how the family meet the criteria (please state the individual the criteria relates to)

  1. Parents and children involved in crime or antisocial behaviour.

A child (under 18) who has committed a proven offence in the previous 12 months
An adult or child who has been involved in an ASB incident in the previous 12 months
An adult who is currently:
  • subject to licence or supervision in the community, following release from prison, OR
  • serving a community order OR
  • suspended sentence OR
  • Returning from custody

  1. Children who have not been attending school regularly.

A child who has any unauthorised absence in the last three consecutive terms
A child who has received a least 3 fixed term exclusions in the last 3 consecutive terms
A child who has been permanently excluded from school within the last 3 consecutive terms
A child who is neither registered with a school, nor being educated in an alternative setting
  1. Children who need help: children of all ages, who need help, are identified as in need or are subject to a Child Protection Plan.

A child who has been assessed as needing early help
A child ‘in need’ under Section 17, Children Act 1989
Young people under 18 years old who have been reported missing at least 3 times in the last 90 days
Children assessed as at risk of child sexual exploitation by TVP or MASH and referred to MARMM
  1. Adults out of work or at risk of financial exclusion or young people at risk of worklessness.

An adult in receipt of an out of work benefit
A young person who is not in education, training or employment (NEET)
A family which has debts or financial problems that impact on their ability to meet basic family needs e.g. food, clothing, accommodation, unable to meet priority bills (rent and council tax) (this would include those who are in work but unable to meet basic needs)
A family who are at risk of homelessness because of a breach of their tenancy agreement
A family who has been in temporary accommodation for more than 6 weeks
  1. Families affected by domestic violence and abuse.

A young person or adult know to local services who has had experience within the last 12 months, OR is considered to be at risk of experiencing, domestic violence or abuse
A young person or adult who is known to local services as having perpetrated an incident or domestic violence or abuse in the last 12 months
The household or family member has been subject to a police call out for at least one domestic incident in the last 12 months where a child 16 and under has been present
  1. Parents and children with a range of health problems.

An adult with parenting responsibilities, or a child, with a drug or alcohol problems
An adult or child with caring responsibilities or who has had an assessment under the Care Act 2015
An adult with emotional and/or social wellbeing and/or mental health concerns who has parental responsibilities
For example, issues relating to:
• Anxiety
• Depression
• Low self esteem
• Low in mood
• Ability to parent children affectively
A child with emotional and/or social wellbeing and/or mental health concerns
For example, issues relating to:
  • Confidence and happiness
  • Resilience
  • Involvement with their peers
  • Ability to build and maintain relationships
  • Learning disabilities
  • Lack of attachment to their carer
  • Tantrums, oppositional and defiant behaviour
  • Speech, language and/or communication difficulties
  • Learning disability
  • Attachment issues
  • Neurodevelopment disorder (e.g. ADHD)
