1. / Welcome, introductions and apologies / Jill Britton
The Chair, Jill Britton (JB)welcomed everyone, stated apologies will be circulated with the minutes and introductions were made.
2. / Minutes and matters arising from the last meeting / Jill Britton
JB went through the actions from the minutes of meeting held on the 05th March 2015:

3. / Carers Strategy / Agnes Olagunju
Agnes Olagunju, LBN Commissioner gave an update on the Newham‘s Joint Carers Strategy whichwas developed between the London Borough of Newham and NHS Newham Clinical Commissioning Group. Furthermore she hasbeen working with the hospitals and community health services to ensure that carers are supported to carry out their tasks less strenuously and to ensure that help is provided where needed.
Agnes encouraged providers to bring forward any ideas and suggestions to support the strategy.
Agnes talked about some of the key messages from carers:
- Personal centred approach
- More use of information and technology
- Less duplication of resources and process
- Clarity on the support available and how to access it
- Improvement in quality of the service.
The main providers of support for carers are:
Newham Carers Network- for Adult Carers
Family Action – for Young Carers (hold monthly meeting/plan activities for young carers)
Agnes asked providers to work together/brainstorm ideas and suggestions on the following questions:
- What do you think Health and Social Care should be doing to improve identification of carers?
- What role do you think providers can play in supporting carers?
- How can we develop the market for carers?

4. / Bowel Cancer Awareness / Leena Khagram - NHS
Leena Khagram, NHS Health Promotion Specialist talked about bowel cancer and bowel cancer screening programme.Leena went through the risk factors, signs and symptoms and the importance of early diagnosis. Please refer to the presentation for detailed information.
Leena provided details on screening test kit and how it is completed. All men and women aged 60-74 are sent a test kit every two yearsthrough the post as long as they are registered with a GP. Leena went through the process and explained that each kit is assigned to an individual and one needs to take three samples to complete the kit. Those who are 75 or overcan call the Freephone helpline number (0800 707 60 60) and request a screening test kit.
Leena explained that her role is to hold workshops in the Community Centres/organisations and she is targeting the carers to raise awareness.Leena requested providers to invite her to their organisation to raise awareness of bowel cancer and agreed to tailor the workshop to meet the needs of different groups. She also said information is available in different languages and the details are on the website.
Some providers raised concerns that they are not receiving the test kit. Leena asked those providers to e-mail her with a list of names, including their NHS number and date of births so that she can request the test kits for them. Leena encouraged providers to email her if they require further information, advice on test kit, clarification about the process and any other questions. 5. / Mental Capacity Act / Deprivation of Liberty Safeguarding (DoLS) / Cathy Newcombe
Cathy Newcombe, LBN Practice Manager for Mental Capacity Act & Deprivation of Liberty said her role is to promote an understanding of the Mental Capacity Act and more specifically to work with colleagues around Deprivation of Liberty Safeguards.
The term “deprivation of liberty” is used to describe individuals who lack mental capacity and whose care or treatment involves a level of restriction or restraint which amounts to them being detained or deprived of their liberty. If this is the case an authorisation should be sought in order to establish whether a Deprivation of Liberty is occurring or going to occur and whether this is in the person’s best interests. A mental health and a best interests assessor is appointed. Their reports and recommendations are scrutinised by a Newham panel with the outcome notified to relevant managers and officers. Cathy is responsible for overseeing the deprivation of liberty process and for providing advice and support on the implementation of the Mental Capacity Act. Decisions on behalf of people who lack capacity should be made in line with the MCA best interest decision making principles. The Deprivation of Liberty Safeguards has been around since 2009, however a there was a major significance change following the Supreme Court Ruling in March 2014 which resulted in a huge increase in Deprivation of Liberty referrals for Newham and accross the country.Newham had a period where it was difficult to respond to requests and completing the assessments in the required timescales, but . with more best interests assessors recently qualifying the service is now managing the process better, Cathy said LBN received 450 applications last year.
Cathy advised providers that it is important to keep records of people that are on the Deprivation of Liberty authorisation. The notifications and the outcome must be kept in individual’s file/folder and keep a seperate record when they are due to expire so that the renewal application can be submitted in good time. It is good practice toreapply 6 weeks before expiry.
Cathy also mentioned that the Supreme Court Ruling made it clear that the Deprivation of Liberty also applies to other settings including Supported Living Providers, Extra Care Housing, Shared Lives and within the Home. Cathy informedproviders to look at the person’s vulnerability and care plans and how they could reduce the restrictions. For individual assessment, providers need to approach the Care Manager and Social Worker or the Access Team to discuss what is required for new referrals.
6. / Safeguarding Update / Tony Pape
Tony Pape, LBN Senior Safeguarding Governance Officer talked about the changes since the implementation of the Care Act such as: updating the policies, reviewing systems and procedures; moving away from safeguarding alerts to safeguarding concerns; safeguarding queries rather than investigation; giving more ownershipon the individual, more emphasis on learning from outcomes.
Tony talked about an engagement exercise last year involving the Health Social Care Information Centre, by looking at customer satisfaction and the outcome process. He also talked about an engagement with the Safeguarding Networking Group to develop a process and he has agreed to share the outcome with the providers. He also talked about a framework that was recently set up to audit the whole safeguarding process , following that, the Safeguarding Team is working closely with the Quality & Workforce Development Team and the Contracts Team to prevent the situation from getting complicated.
Tony is happy to answer any questions and is also willing to visit providers to discuss any processes and changes and asked to contact him through e-mail (). Tony reminded everyone about the safeguarding training that is available especially for managers which is listed in the Workforce Development Newsletter He emphasised the importance of this training as it provides an insight into how safeguarding should be dealt with in certain areas. He reiterated providers should make use of this free training offered by LBN.
Tony said, the Safeguarding Adult Partnership Board recommended changes from the Care Actwhich gives local authorities more ownership in undertaking Safeguarding Adult Reviews and are currently processing two cases as a result. He went onto say, it is a lengthy process and the outcome along with lessons learned will be shared. Tony said, the Care Act made the safeguarding process much simpler and reminded providers to access the information online.
A provider asked whether the Pan London Safeguarding procedures are being updated. Tony confirmed that they were due to be finalised in September with the launch in the new year. Outcome of the survey recommendations are with ADASS and an update may be available at the next meeting.
Tony reminded everyone that the safeguarding line 0203 373 0440 is open from 9am till 5pm from Monday to Friday which is answered by the Access Team and then transferred to the appropriate safeguarding teams (Supporting Care and Safeguarding Governance). During the out of hours, calls are answered by Telecare.
Another provider asked whether the safeguarding form has changed. Tony confirmed that this was the case and advised providers to use the new form to raise any futureconcerns.
Jill Britton asked whether the providers are receiving responses to safeguarding concerns as this is an issue raised at one of the previous meetings. One of the providerssaid they are still not receiving responses. Tony confirmed responses should be received and a copy of an outcome report is to be shared with the person who raised the concernonce the investigation is completed. Tony advised providers to be proactive and chase the responsible person rather than waiting for forums such as this to raise their concerns. Tony reminded that Doreen Williams, Team Manager and Scaria Joseph, Practice Manger are responsible fordealing with Safeguarding issues raised with the Supporting Care Team.Failing that, if you receive an unsatisfactory response, speak to Tiffany Adonis-French, Service Manager.
Moira Storrar, LBN Quality and Workforce Development Officer said the Quality & Workforce Development Team are providing training to all LBN managers on how to deal with Safeguarding concerns; and once everyone has completed the training, it will give an opportunity for providers to evaluate the effectiveness of the training.
7. / Workforce Development Update / Moira Storrar/ Angela Skrabania
Moira Storrar, LBN Quality and Workforce Development Officer said she put together a Newsletter as she has a lot of information to share with providers (this will be circulated with the minutes).
Moira stressed that in the last finacial year 2014/15, over £7,900 was lost from the limited budget on the multi-agency safeguaring programme alone from PVI organisation for late cancellation and non attendance. Therefore, LBNhas enforced cancellation fees from this financial year 2015/16 and already started sending out invoices for the 1st quarter.
Moira reiterated that it is the responsiblity of the delegate and their manager to inform the Quality & Workforce Development Team () if a worker is unable to attend the training stating the reason. If the reason is a real emergency i.e. accidents, illness, bereavement etc it will be dealt with sympathetically. Moira informed providers are expected to show some compassion and also equally take responsibility in informingthe Workforce Development Team by telephone or e-mail or as soon as possible.She reiterated that if the reason for cancellation is an emergency, providers will not incur the cancellation. Moira said some of you may have already completed the Skills for Care Survey Monkey questionnaire and theWorkforce Devlopment team have received the results. One of the issues that was identified is how hard it is to engage peoplein workforce activitites, even if there is no cost attached. Another Survey Monkey will be sent to our localproviders with the aim to get real ideas to get together to work better and share resources. Skills for Care have put out a lot of free training, but has been poorly attended.
Moira asked to let her know if providers have any information aroundThe Care Certificate, whether there are any problems, successes etc. Moira reminded that there is information regarding The Care Certificatecontained in the Newsletter.
Angela Skrabania (Senior Practioner for Assisting and Supporting Individuals)said LBN at theconception stage of thinking about an assisting and moving project and would like providers to be involved in this project.Angela would like toworktogether to think about how training should be delivered to develop both an understanding andexpertise in this area.
Looking to create some learning hubs across the borough where there is up to dateequipment and where resources are accessible totrain staff members.
The Quality & Workforce Development are looking to deliver accredited training coursesspecifically on assisting and moving. Following this training you will then be able to deliver this training in-house. Angela urged providers’ involvement in this. Angela said she is keen to push this forward and cannot do this alone and asked providers to contact her via email ()in order to get together possibly around September/October to work in partnership.

Jill Britton said those providers who are on the ILSS Framework should have received a one year extension to the contract and also started the procurement process. Next week, there are two Market Engagement Days at Stratford Town Hall, Thursday 6th August and Friday 7th August. LBN will be using a different evaluation process and looking to start the tender process around October 2015.
