STATEMENT OF WORKLOAD FOR (FACULTY MEMBER - NAME)

The total work assignment will be .5 FTE (50%).

After Fall 2010, phased retirement workload will be determined annually by the faculty member and the dean or the dean’s designee during the remainder of the phased retirement appointment, and to which the University must be agreeable.

a.Faculty Member presently holds the ______Professorship/Chair at the College of ______, for which Faculty Member receives a salary supplement in the amount of ______Dollars ($__,___.__) per fiscal year. It is Faculty Members desire, and the University’s present intention, that Faculty Member continue (OR not continue) to hold the ______Professorship/Chair during the period of the phased retirement appointment, and receive a salary supplement in the amount of ______Dollars ($__,___.__) per annum from the income from the endowment of said Professorship/Chair during that period.

b.If, for any reason, Faculty Member ceases to be the holder of said Professorship/Chair during the period of the phased retirement appointment, then the salary paid under paragraph a. above shall beaugmented by the sum of ______Dollars ($_,___.__) per annum, or applicable portion thereof, during the period of the phased retirement appointment. The terms and conditions of the phased retirement appointment may only be changed upon mutual written agreement of Faculty Member and University acting by and through the Faculty Member's unit administrator and dean.
