ADURS 2014 Symposium

To encourage maximum attendance at the 2014 ADURS Symposium, a major portion of the expenses of the Members will be defrayed in the following manner:

1. REGISTRATION FEE

There is no registration fee for ADURS-funded attendees (State Delegate/Voting Member; Alternate State Delegate, Program Committee Member, Speaker, Director, Staff, or other as determined by ADURS).

$500 for non-ADURS funded attendees (registration fee covers educational programming and ADURS meals – cancellations must be made by January 21, 2014 for a full refund – no refund will be paid if after this deadline). Fee may be remitted by check to ADURS, Inc. 3006 Edinburgh Drive, Edmond, OK 73013. Credit card payments will be accepted via PayPal on www.adurs.com during the registration process.

IMPORTANT – ADURS TRAVEL AND LODGING POLICY

If attending as a state representative (voting or non-voting) with ADURS paying for travel, hotel and registration, you are agreeing to the following:

·  You will attend all programs (lectures) presented at ADURS

·  You will attend the business meeting if you are the voting member

Failure to attend all required aspects of ADURS without appropriate excuses (we are fully aware that there can and will be times where job obligations will cause conflicts – simply let ADURS staff know if this occurs) will result in this being reported to the ADURS Board of Directors for review. The Board may choose to revoke the sponsorship and invoice you for all costs of your attendance (travel, hotel and registration) at ADURS. If such action is taken, your supervisor will be notified.

2. HOTEL ACCOMMODATIONS

ADURS will pay for a three night stay (February 20-23) for all ADURS-funded attendees. Accommodation for a three night stay (Thursday-Saturday), single rate, plus any applicable taxes can be made for each State or Alternate Delegate attending the Symposium at:

Any reservation changes/requests/additions received after the deadline

will be accepted on a space-available basis at the hotel’s discretion.

This year you will be able to register online by clicking on the web page address and using the group name and code. However, you may also call the hotel directly for your room reservations, and specify that you are attending the ADURS Symposium. You will be asked for credit card information in order to receive a reservation confirmation number and your credit card will be billed for incidental charges you may incur and/or extra days stay.

If you desire to arrive in Scottsdale before February 20th or depart after February 23rd, lodging will be at your expense. Unless sponsored/funded individuals have prior approval from ADURS, your lodging will only be paid by ADURS for the three nights of February 20 - February 23, 2014.

YOUR SPOUSE/PARTNER may attend the ADURS Symposium for an additional fee of $250.00 if participating in all ADURS meal events or $100.00 for Thursday reception buffet. There is no additional charge for hotel; airfare is not included. Fee may be remitted by check to ADURS, Inc. 3006 Edinburgh Drive, Edmond, OK 73013. 3. TRAVEL: Air and Ground Transportation

Air Transportation:

a. All arrangements for ADURS-funded attendees must be made through the ADURS travel agent, Journey House Travel, at 405-463-5800 (ask for Marta) or email Marta at: . You must specify that you are attending the ADURS Symposium. Spouse/partner travel and payment arrangements may also be made with Journey House at your own expense.

b. Airfare purchases by ADURS-funded attendees or their employers are not reimbursable (unless prior authorization has been made with Deanne Jacobs).

c. ADURS-funded attendees traveling by car must obtain prior authorization from ADURS to receive mileage and parking reimbursements.

d. All ADURS-funded attendees must make their travel arrangements by January 21, 2014.

Ground transportation is at your own expense.

The 2014 ADURS Symposium is designed for pharmacists specializing in medication utilization within State Medicaid programs and is sponsored by the American Drug Utilization Review Society (ADURS) and the Nebraska Council for Continuing Pharmacy Education (NCCPE). NCCPE is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. Credit for courses must be earned by the participant. Participants will earn up to 12.50 contact hours of continuing pharmacy education (CPE) credits for attendance of the entire symposium.

In order to earn CPE credits, the participant must complete an on-line educational evaluation form for each CPE session they attend within 15 days from the end of the symposium. The evaluation will collect their birthdate (MMDD) and NABP e-Profile ID number (to receive CPE credits, the attendee must have an NABP e-Profile ID number).
