STATE 4-H AWARD SELECTION
COUNTY TRANSMITTAL FORM INSTRUCTIONS
Due with State 4-H Award Applications - January 8, 2016
To State 4-H Office
FOUR Award Applications are due on the January 8, 2016 deadline. This transmittal form MUST BE SUBMITTED with ANY of these applications, in one way or another. Here are the submission details on each type of application:
· NEW Illinois 4-H Award Applications – Using the NEW applications provided in October, applicants may apply in any/all of five new award areas: Community Service, Communications, Leadership, Personal Growth, and Project Mastery. These applications must be SCANNED AND EMAILED to by 5:00 p.m. on Friday, January 8. This transmittal must be emailed to this address as well at that time. Please follow these instructions:
o Email Transmittal first. In the Subject line of the email, type Award Transmittal - ______County.
o Scan and Email the complete application and ALL essays from a single individual AS ONE FILE. (For example, if a member is applying in both Communications and Project Mastery, the scanned file should include the front “demographic/signature” page, 2 pages with Sections 1, 2, & 3, and four essays (2 essays for Communications and 2 essays for Project Mastery.) In the Subject line of the email, type in the member’s name and number of categories entered (i.e. Susan Kool – 2 Categories).
· 4-H Veterinary Science Award Application – Scan and email the complete application as one file and email. In the Subject line of the email, type in the member’s name and “Vet Science Award.” Information should be included on this transmittal and emailed.
· Illinois 4-H LCP Applications – This application must be MAILED, postmarked by January 8, 2016. Since the application contains sealed envelopes with Reference Evaluations, we wish to receive those evaluations in their original envelopes with the evaluator’s signature across the seal. Maximum of two applications per county. Information should be included on this transmittal and emailed (Even if you are only sending in this award by mail, the emailed transmittal will let us know it is coming.)
· Illinois State 4-H Key Award Applications – This application must be MAILED, postmarked by January 8, 2016. Since the application contains sealed envelopes with Reference Evaluations, we wish to receive those evaluations in their original envelopes with the evaluator’s signature across the seal. Maximum of two applications per county. (Even if you are only sending in this award by mail, the emailed transmittal will let us know it is coming.)
Special Notes & Reminders:
¢ Members must be 15-18 years of age as of September 1, 2015 in order to submit Illinois 4-H Award Applications. Applications for the 4-H LCP Award must be current 4-H members age 17-19 as of 12/1/2015. Applications for the State 4-H Key Award must be 16-18 years of age as of 9/1/2015.
¢ There is NO LIMIT to the number of State 4-H Award Applications an Extension Unit may submit EXCEPT for the Illinois 4-H LCP and State 4-H Key Award applications. For the 2016 Illinois 4-H Award Applications, ANY 4-H member of eligible age from that unit may submit their applications without the nomination process. However, applications must still be sent via the local Extension office and include signatures from an Extension staff member.

This award requires a special application, plus a minimum of 3 Reference Evaluations. The application can be found on the 4-H Contacts Portal (Awards, Scholarships & Recognition = Awards Program & Applications). Members must be at least 17 years of age by December 1, 2015. Counties may submit up to two applicants (per geographical county).

Applications for Illinois 4-H Veterinary Science Award
This award requires a special application. The application can be found on the 4-H Contacts Portal (Awards, Scholarships & Recognition = Awards Program & Applications). Members must be 15-18 years of age by September 1, 2015, must be currently enrolled in the Veterinary Science project; and must have been enrolled in the Veterinary Science project at least three years (current year included). There is no limit to the number of applicants per county. Additional names can be added to the last page of this transmittal.
