CARE STAFF DAILY RESPONSIBILITIES

G / Night RN handover to Day RN – using Residata ‘End of Shift Report’ only.
RN to handover to care staff any changes to care being given to residents.
RN to allocate care staff accordingly ensuring breaks are designated and any escort duties assigned.
Request from care staff weight to be taken for scheduled resident due for review.
Ensure mealtimes are co-ordinated by designating staff their responsibilities at these times.
Discuss with social co-ordinator plan of activities for the day.
RN to check diary and action any entries – prioritise and complete before end of shift.
Complete medication round with med-carer when available.
Provide all clinical care.
Review resident’s charts ensuring they are completed and sign to evidence this check. / Continue with personal care of residents from the night staff.
Determine your allocation from RN and ensure changes to care are understood and carried out.
Ensure residents are offered a choice of breakfast and are supported to enjoy this mealtime either in their rooms or in the dining room.
Serving area is kept clean and tidy.
Residents charts to be completed.
Any concerns with regards to resident’s wellbeing should be reported to the RN for appropriate action.
Room checks to ensure beds are made and rooms are tidy.
Yellow bags are monitored and disposed of throughout the shift.
Fluid intake is encouraged by offering tea, coffee, cold drinks and smoothies throughout the shift.

E / Ensure care staff are assigned to assist residents at mealtimes.
Complete medication round without med-carer and ensure any medication shortages have been actioned.
Review resident’s charts ensuring they are completed and sign to evidence this check. / Ensure dining area is set and prepared for lunchtime meal and the TV is not on during mealtimes but either resident’s choice or easy listening music is played.
Clean and tidy serving area after meal and ensure all cutlery and crockery is collected from rooms and returns to kitchen.
Ensure ALL resident’s charts are completed.

N / 2.30pm Care Staff Meeting led by RN (leaving at least one Carer with residents) to review all aspects of resident care.
Review care plan with 1-2 carers as per schedule.
Do a supervision with a carer as per schedule.
Prompt care staff to encourage resident’s participation in activities. / Begin input of daily notes on Residata.
Promote participation of activities.
If able then attend care meeting at 3pm
Collate meal preferences for next day and inform the kitchen.
Prepare dining area for Teatime.
Ensure all crockery and cutlery is returned to the kitchen including those in rooms.

Throughout the shift constant observation of each resident should be noted and any concerns reported to the RN for appropriate action.

The promotion of both food and fluids should be encouraged, regardless of set mealtimes or drink rounds provision of snacks and drinks should be offered throughout the shift.
