V. Terms & Conditions條例及規則:

  1. Completed application forms should be submitted to the main reception of Discovery College.
  2. Class allocation is subject to a first-come first-served basis. However, priority will be given to DC students.
  3. The program is organized on a term basis that will be confirmed by the instructor, no phone call means your application is successful. Please just show up at the chosen time.
  4. A cheque with the correct amount of the course fee must be attached with this application form when submitting to the College’s main reception. Otherwise, no enrolment will be accepted. Please make cheque payable to:
    “Sportsmanship International Ltd.”
    Please write the Participant’s name on back of cheque.
  5. After confirmation of place, failure to attend the lessons in no way absolves the participant from the financial obligation. NO refunds or credit will be provided for students withdrawing after enrolling for classes.
  6. Lessons may be cancelled in the event of Pandemic, RED and BLACK storm warnings, or the hoisting of the number 8 Typhoon signal or the instructor decides adverse conditions warranting the cancellation of the lesson in according with ESF policy. If there is a possibility of lessons being cancelled due to the above reasons, please call3969 1000 for information as close to your lesson time as possible but within normal office hours (8am-4pm).
  7. Lessons cancelled due to the adverse weather will not be rescheduled.
  8. Sportsmanship International Ltd. and Discovery College reserve the right to change course venue and time without prior notice.
  9. No lessons on Public Holidaysunless specified. Please check time, date and venue carefully when enrolling for classes.
  10. Helpers and lesson companion may accompany children at the Management’s discretion but must leave the venue as soon as the lesson is finished.
  11. If your child suffers from any illness that may require attention during the programme, please make a note on the application form.
  12. The appointed instructor will take every precaution to prevent accidents.

VI. Declarations 聲明:

I agree to allow the participant to participate in the Gymnastics Program and confirm that the information given above is true and correct. If this application is successful, I shall be fully responsible for the payment of all the expenses and be bound and abide by the rules ofDiscovery College. Sportsmanship International Ltd., Discovery College, its holding companies, their officers, employees, servants, agents or licensees, shall not be liable or responsible to the participant and/or the lesson companion for any loss, injury, damages, claims, cost or expenses whatsoever, which may be sustained by the participant and/or the lesson companion, caused by or in consequence of the Gymnastics Program. I fully understand that Gymnastics is a high risk activity. I declare that the participant is healthy, physically fit and suitable to participate in the above-mentioned activity. I also fully agree to the terms and conditions above in this application.

We urge parents to carefully interview and check reference before enrolling. Distribution of materials is simply for informational purposes and does not constitute a recommendation or endorsement.
