STAFF EXIT CHECKLIST
STATEMENT
Employees ceasing service with Ermha are required to complete Staff Exit Checklist with Program Manager or Supervisor before close of business of their final day of employment. All relevant items on checklist must be addressed and failure to complete or return property of Ermha may result in delay of Employee’s termination payment
