Guidelines for completing Question & Group Presentation form

Individual Responsibility: On completion of successfully passing your 2 FHC Canadian field assessments you will have 6 months to ensure that Individual Questions and Group Presentation information is completed, signed and returned. Ensure you print off the above form.

Individual Questions Section

1. Complete name and provide details of where the questions took place as per form.

2. List who the members of your Question panel are and the questions asked.

One Panel Member where possible should be or have been a Canadian rated umpire - or be an experienced Regional umpire - or have thorough Technical Table knowledge.

At least 2 relevant umpiring scenarios, field hockey rules or game management questions should be asked but is not limited to this.

3. One Panel Member to complete confirmation of competency then sign and date.

Group Presentation Section

1. When giving your Group Presentation/Discussion/Talk it should be at least, but is not limited to 15 minutes. The topic should be field hockey related. (e.g. - experience of umpiring at first tournament, discussing a specific rule, the challenges of umpiring, etc). The Presentation can take place in or out of your province at any of the following opportunities but is not limited to – (if other please detail this on the form):

At an Introductory Rules Clinic (for club, junior/high school level, etc)

At a Parents Rules evening

At an Elementary, Junior or High School class

At Community, Provincial or Regional Umpiring Course

2. Complete details of location, date, etc.

3. Provide brief details of what you presented.

4. Request witness confirmation that you completed the Group Presentation/Topic discussion

Witness should be Course Facilitator/Umpire/Technical person/official Club member/ PSO Umpiring Coordinator/Teacher/ etc.

5. Return the original form to FHC via email or by post:
