I.SCOPE OF SERVICE
The Department of Human Services is soliciting for a contractor to provide a diesel generator to Wernersville State Hospital(WeSH), located at 160 Main Street Wernersville PA 19565. Specifications are attached.
II.BIDDING INSTRUCTIONS

- All services provided by the bidder shall be subject to review and approval by the Contract Administrator / Monitor.
- Entering pricing on the Invitation for Bid – “Quantity” on the bid is an estimated quantity or units. Bidders should enter their price for each line or category listed in this Invitation for Bid.
VINVOICING
Payment Provision: The vendor will be reimbursed only for services and parts accepted by Wernersville State Hospital.
The vendor shall be paid upon satisfactory completion of work performed and submission of an invoice on the vendor’s letterhead.

VI.GENERAL INFORMATION
- Commonwealth of Pennsylvania Business Associate Appendix Language Health Insurance Portability and Accountability Act HIPPA) Compliance applies to this contract.
- The contractor is responsible to carry all necessary insurance and liability coverage to cover this service and provide certification of such insurance.
It shall be understood and agreed that any quantities listed in the proposal are estimated only and may be increased or decreased in accordance with the actual requirement of Wernersville State Hospital and that Wernersville State Hospital in accepting any bid or portion thereof contracts only and agrees to purchase only the quantities as represent the actual requirements of Wernersville State Hospital.
