Each student should be allowed to provide in-person contact with clients and constituencies at their site, for a minimum of 50% of their total required field hours. In-person contact refers to interpersonal interactions with clients and constituencies, and may include the use of digital technologies. Please provide an estimate of the number of in-person contact hours with clients and constituencies the student may expect to complete during the course of this internship.

NOTE: Signature indicates collaboration and agreement with the learning contract, which contains the broad competencies required of master’s-level clinical social workers by the Council on Social Work Education (CSWE). The student learning contract provides a broad framework for developing an individualized learning plan which is responsive to the needs of the student and the resources of the field agency. Please feel free to develop creative opportunities at the agency and utilize other community resources, if necessary, to provide opportunities such as work with groups, which may not be available in every practice setting.
This document also serves as the basis for mid-term and final evaluations. These evaluations are completed separately. The format for evaluation is as follows:

Competent performance is demonstration of knowledge, values, and skills where all components of the practice behavior are included, but at the beginning or the rudimentary level.
2 / Inadequate Performance / The intern/student shows beginning application of the knowledge, skills or dispositions related to the performance of the practice behavior. / Inadequate: “Failing to reach an expected or required level or standard”. Inadequate performance is demonstration of knowledge, values, and skills where one or more of the components of the practice behavior are missing.
1 / Lacking Performance / The intern/student has not demonstrated application of the knowledge, values, and skills related to the performance of the practice behavior. / Lacking: “Missing, not present or available”. Lacking performance is the inability to demonstrate any of the components of the knowledge, values, or skills related to the practice behavior.
N/O / Not Observed / The intern/student has not had the opportunity to demonstrate the knowledge, values, and skills related to the performance of the practice behavior. / Observed: “To see or notice something, especially while watching carefully”. There was not observation of the performance or practice behavior.
Note: By the end of the year, the student is expected to demonstrate overall proficiency in each competency that is evaluated, i.e., a rating of “3”. Ratings of “4” or greater are not expected of every student and should be reserved for truly exceptional performance. All ratings should be substantiated through specific examples.
