Science Safety Contract

Science is an active, hands-on class. We will be doing many activities that require the use of sensitive equipment and potentially hazardous chemicals. Safety is the #1 priority in our classroom. To ensure a safe learning environment, all students will be instructed in science classroom safetyand the student/parent/guardian safety contract will be signed and filed at school. In order to enjoy continued involvement in lab-related activities, students must promise to adhere to the following rules:

1. Act responsibly at all times in the classroom. Horseplay, jokes, and pranks are not appropriate and will not be tolerated in a science classroom.

2. NEVER eat, smell, or place your face over any of the supplies unless instructed to do so by the teacher.

3. Follow all instructions, written and verbal, about the laboratory procedures given by the teacher and in the lab.

4. Be properly prepared to do the experiment. Gather all materials, read all instructions, and listen to all instructions BEFORE starting the experiment. Lack of knowledge makes you dangerous to everyone in the class. If you have questions, ASK before you DO!

5. Wear appropriate protective equipment when directed to do so. This includes aprons, goggles, heat, and/or chemical gloves, etc.

6. Tie back long hair, baggy clothes, and dangling jewelry while doing a laboratory activity.

7. Learn the location and operation of safety equipment. This includes eye washes, fire extinguishers, fire blankets, and first aid supplies.

8. Eating, drinking, or chewing gum in the lab is forbidden, unless told to do so by the teacher.

9. Turn off all burners or hot plates when not in use. NEVER leave them unsupervised.

10. Notify teacher of any accidents/injuries or misuse of equipment, no matter what and as soon as they happen.

11. Treat burns immediately by running cold water on them for 15 minutes or more.

12. Read chemical labels carefully. Read the label at least three times:

a. Read the label when you pick up the chemical.

b. Read the label just before you obtain the amount you need/use it.

c. Read the label after you are finished.

Mixing the wrong chemicals is dangerous.

13. Dispose of all chemicals carefully. Follow the directions given in the lab information.

14. Never return unused chemicals to their original (stock) containers.

15. Wash off all chemicals splashed or spilled onto your skin or body immediately for 15 minutes or more. Remove contaminated clothing immediately and notify your teacher.

16. Keep your lab and personal area clean. Clean up all spills as soon as they are made. This includes water.

17. List any allergies or medical concerns at the bottom of this page. Know what you can and cannot handle so you can alert your instructor.

18. NEVER take chemicals, equipment, or supplies out of the lab. This constitutes THEFT and is a crime.

19. Clean up your lab area. Put away all supplies in their correct place. Wash your hands after every lab.

Consequences for Infringing Upon This Agreement

Because of the liability and safety issues concerning lab-related activities, the teacher’s foremost responsibility will be to keep students safe. This is a serious concern, and should students choose to violate this agreement, the following actions will be taken:

Violation #1: Student may be removed from the lab and sent to the main office for disciplinary action.

Violation #2: Student may be removed from that lab and EVERY lab until parent(s) come in for a conference with the science teacher and an administrator.

Violation #3: The student may be removed from ALL LABS FOR THE REMAINDER OF THE SEMESTER. Alternative written assignments may be provided by the science teacher so the student may earn academic credit.

Remember; do not tear off the signature portion of this letter. Return it completely intact.
