TO REGISTER

To register as a patient obtain forms from reception. Documentation of proof of address, current within the last 3 months, as well as photographic evidence is required. All patients are asked to attend a health check with the Healthcare Assistant/Nurse (and to also see a doctor if they are on regular medication). Please inform the receptionist if you wish to register with a specific doctor.

PREFERENCE OF PRACTITIONER

All patients are allocated a named accountable GP. Having a named GP does not prevent you from seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor. If you insist upon waiting to see a specific doctor then this will depend on that doctor’s availability and may result in you having to wait longer than the 48 hours anticipated in the NHS Plan.

Where patients are requesting to join the practice list, the practice does not discriminate on the ground of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

CONSULTING ARRANGEMENTS

Patients are seen by appointment. Appointments can be made by either telephoning any surgery. Urgent cases will be seen as soon as possible and always the same day, although this may not be with the Doctor of your choice.

TRAINING

The practice has been recognised for the training of future General Practitioners. Dr Borrill has been approved as a trainer in general practice and Dr Platts as a family planning trainer. The Registrar is a qualified doctor who has chosen a career in family medicine and will be one of tomorrows GPs. Consultations may occasionally be video recorded with patient consent. Your help in training is invaluable.

HOME VISITS

If you feel you need a home visit please contact Epworth surgery, wherever possible before 10.30 am, where the duty doctor will assess your request.

NIGHTS, WEEKENDS AND PUBLIC HOLIDAYS

If you require medical attention in the case of an emergency out of surgery hours please ring NHS ‘111’. You will be offered advice, an appointment to attend the out of hours centre, or the duty staff will assess the need for a home visit on medical grounds. The call is recorded for medico-legal reasons. NHS North Lincolnshire CCG is responsible for commissioning this service. If you require general medical advice please ring NHS ‘111’.

FACILITIES FOR THE DISABLED

All surgeries have been assessed and deemed to offer suitable facilities for the disabled. If you are unable to manage the stairs at Epworth surgery please tell the reception staff when you make your appointment – a doctor will be able to see you downstairs.

DISPENSARY AND REPEAT PRESCRIPTIONS

If you are on regular medication your doctor will arrange for you to receive it on a repeat prescription. Remember to give 48 hours notice for your prescription to be prepared. Patients living more than a mile from the Pharmacy in Epworth can have their medication supplied from one of the surgeries. Prescriptions can be ordered at any of the surgeries.

SERVICES UNDER NEW GMS

The Practice provides a full range of services under the GMS contract. As well as providing essential services, the Practice offers the following additional services:

Ø  Vaccinations and immunisations, including childhood immunisations and travel

Ø  Child Health Surveillance

Ø  Family planning

Ø  Maternity services

Ø  Minor surgery procedures

Ø  Health promotion

Ø  Continuing care

Ø  Influenza immunisations

Ø  Physiotherapy clinics

In addition, the Practice has chosen to provide the following enhanced services:

Ø  Primary Care Access – the Practice continues to work towards identifying and implementing approaches to shaping the demand for face-to-face consultations

Ø  Extended Minor Surgery

RIGHTS AND RESPONSIBILITIES

You have the right to be treated with respect and without discrimination but certain responsibilities are also expected of you – for example if you are unable to keep an appointment please cancel it to ensure it can be made available for another patient to utilise.

ZERO TOLERANCE

Patients who are violent and/or abusive to doctors, staff or persons present on the practice premises may be reported to the police and may be removed from the practice list.

COMPLAINTS

The practice has a complaints policy. Complaints should be made in the first instance to the Reception or Dispensary Supervisors, where appropriate. If this is not appropriate, complaints should be in writing addressed to the Practice Manager.

PATIENT INFORMATION

The doctors and staff have access to patient information but this cannot be divulged to a third party without the patient’s written consent.
