Chapter Award
The Chapter Award honors the chapter that best promotes the goals of SVN through their member relationships, community activities and promotion of community health issues. All chapters are encouraged to submit an application for this award.
Eligibility Requirements
v The chapter must be officially chartered by the SVN Board of Directors.
v The chapter must be in compliance with all chapter obligations, and in good standing with the SVN National Office.
v The chapter must submit a complete application by the stated deadline.

1. The Chapter President will confirm their acceptance in writing to the National Office.
2. The Chapter President (or designee) will be present at the 33rd Annual Convention to accept the Chapter Award.

Chapter Award Criteria
/ Criterion Met as Evidenced by:Chapter Management (20 points)
Are the chapter president’s expectations and goals clearly communicated to committee chairs and board of directors?
Are all chapter office/board positions currently filled?
Is the current list of chapter officers currently on file and up-to-date at the National office?
Are the treasurer’s records audited annually (either formally or informally)?
Membership Development (20 points)
Do you have a system for reminding members to renew their chapter memberships?
Do you have a welcome program for new or transferred members?
Do you promote certification or re-certification activities for chapter members?
Do you engage in recruitment activities or have a formal plan?
Member Relations (20 points)
Do you provide approved contact hours for education offered at meetings?
Do you publish a chapter newsletter?
Does your chapter have funding assistance for members to attend school, seminars, or the annual conference?
Does your chapter participate in National SVN activities?
Community/Health Promotion Issues (20 points)
Is your chapter involved in community education or research projects?
Does your chapter participate in or collaborate with others involving research to establish best practice for care of your community vascular patients?
Does your chapter work with other state nurse associations or policy makers on issues involving vascular patients?
Does your chapter offer an educational offering on current or pending legislation associated with vascular issues?
Special Committee Activities (20 points)
Does your chapter have one or more active specialized committees?
Has your chapter or a chapter member presented a paper
or poster at a national conference related to these activities?
Has this paper or poster been published in JVN or another journal?

Chapter Award Criteria

Are the chapter president’s expectations and goals clearly communicated to committee chairs and board of directors? / Quarterly planning meeting of officers.
Are all chapter office/board positions currently filled? / List of officers.
Is the current list of chapter officers currently on file and up-to-date at the National office? / Attach copy of your annual report.
Are the treasurer’s records audited annually (either formally or informally)? / Attach audit report.
Membership Development (20 points)
Do you have a system for reminding members to renew their chapter memberships? / Announcements at annual business meeting. Mailings to home address.
Do you have a welcome program for new or transferred members? / If yes, explain. Attach sample documents, if applicable
Do you promote certification or re-certification activities for chapter members? / Study groups. Review sessions. Describe
Do you engage in recruitment activities or have a formal plan? / If yes, explain.
Member Relations (20 points)
