Dear Pharmacist

SLA 2017/18 for PGD (Varenicline & Bupropion) and NRT voucher scheme

I enclose for your attention the renewed PGD for Varenicline, NRT Voucher scheme and the new PGD for Bupropion (Zyban). Please note that the PGD for Varenecline now reflects the removal of the black triangle and amendments to the restrictions for use following the Eagles Study. (See page 2-3 of PGD).

I hope that your pharmacy continues to participate in the above schemes. As indicated above there is now a PGD for Bupropion (Zyban) and this is also included in the pack.

We would be grateful if you would sign the PGD for Bupropion as this enable all smokers to have access to the full range of medication available to assist in quitting. If your pharmacy currently only operates one scheme and you would like to participate in all schemes then please complete the appropriate section of the SLA . t and return the appropriate section to the service at your earliest convenience.

If you should no longer wish to participate or have ann questions or concerns then please give me a call on the above number.

Reminder:

All vouchers must be entered onto quit manager by the 5th of the month. Failure to do so will cause a delay in payment to the pharmacy. Pharmacies that enter vouchers more than 12 weeks from the date items were dispensed may be subject to payment not being made. This will be at the discretion of the service.

Please can we remind pharmacies that only vouchers issued in Sheffield can be dispensed in Sheffield. Should vouchers be dispensed from other areas of the wider Yorkshire Smokefree Service such as Barnsley or Doncaster then payment will not be made.

If a named person responsible for entering the vouchers should leave the pharmacy then please notify us immediately so that we can update our records and another password can be allocated.

Locum pharmacists can dispense against the PGD as long as they read and signed the document. This document should always be left in the pharmacy and be visible and accessible at all times.
