The purpose of this questionnaire is to get feedback from as many residents of the Forest Park neighborhood as possible regarding their perceptions of traffic safety issues on the public roads in our neighborhood. Your feedback is vital to better identify the severity and location of traffic problems, to gauge neighborhood support and involvement in combatting those problems, and help county agencies put measures in place to mitigate them. Please take the time to fill out this important survey and return it via mail or email to .

Directions

In the table below, list the traffic safety problems that you or others in your household have experienced on public roads in the neighborhood. Please indicate the road segment, problem code, problem severity, time of year and time of day when you experience the problems. List problems separately if they have different severities, seasons or times of day. Then please answer the questions on page 2 of the form.

Road segment or location: What part of the road?, e.g. Skyline north or south of Germantown. Or list a specific location like an intersection, block number or building (e.g. Skyline Tavern).

1. Do you have a good location on your property where the Sheriff’s Office could set up its radar trailer to monitor vehicle speeds for a set period of time? If yes or maybe, please answer #5 below.

2. As one possible solution, would you support having posted speed limits on Skyline Blvd. or other major roads where there currently are none?

3. Do you think the neighborhood should consider raising funds to purchase its own radar unit (pole-mounted or hand-held) for traffic speed calming and monitoring?

4. Would you support the possible creation of an Enhanced Sheriff’s Patrol District for the Skyline area to increase patrols similar to one in Washington County?
