SKI AND SNOWBOARD APPLICATION 2016
Sugar Salem School District will offer a ski/snowboard instructional program again this year for students from Kershaw, the Junior High and the High School. We can transport approximately 200 people including bus directors so registration will be on a first come-first serve basis.. You can also download this application and information forms on the district web site at www.sugarsalem.org/district/forms. The dates of this year’s program are Fridays, Jan.8th, 15th, 22nd and 29th. Students will leave from their school at 2:00 pm and return to the JH at approximately 8:15 pm. Jan 29th is a teacher-inservice day with no school for students. We will meet at 1:30 that day so we can get everyone’s gear loaded and up to Kelly Canyon in time for lessons.

The cost of the program includes:
Transportation from the schools to Kelly Canyon and back to the schools
Lift pass for four hours
1 hour group Ski/snowboard lesson from certified Kelly Canyon Instructor
4 weeks of learning a lifetime recreational skill while having fun!
All ski school participants must ride up to Kelly Canyon on the bus. NO FOOD may be eaten on the busses for JH and HS students. If students eat on the bus, we will ask their parents to transport them the following week. The students will have time to eat in the lodge either before or after their lessons. They have also requested NO COMPUTERS on these trips. The bus will not be secure and the machines could easily get damaged. Students will have to make arrangements to leave them at the high school or with friends/family.

For 2016 Kelly Canyon has asked that we not use the Idaho Parks and Rec 5th/6th grade passes for ski school. FYI-if you go to www.skiidaho.us you can get the info for the 5th grade passes (3 lift tickets) and also a 6th grade pass (2 lift tickets) for some full day passes that you can use at another time.

Your student WILL NOT be able to ski or snowboard without this documentation.
Waiver of Insurance
I, the undersigned parent or guardian of the registered student, hereby release Sugar Salem Ski School, Kelly Canyon, and the Sugar Salem School District, its supervisors and instructors from any responsibility for any accident incurred in this program. This will include any accident in the ski area or while traveling to and from the ski area.

I understand that transportation will be provided to Kelly Canyon and back to the Sugar Salem Junior High School. Students may not be transported by other students! I agree to pick up or arrange transportation for my son/daughter from the Junior High at the scheduled arrival time.
