Parent / Student Agreement

*Parents and teen should read this entire page and sign the bottom. By signing this form you are telling us that you have read and agree to all the information below.

*Late Night Lock-In Rules & Regulations: If you have questions or concerns please contact Mrs. Linda Lietaert at .

*The event begins @ 5 pm on Thursday, June 29, 2017. Teens will meet in the teacher’s lounge at the school. The evening will include mass/prayer in the chapel and the remainder will be in the school. The Lock-In ends on Friday, June 30, 2017 @ 12am. Students who are driving should park in the lot closest to the teachers’ lounge. Parents who are dropping off and picking up should meet their teens at this door.

*No food or drink should be brought to the Lock-In. All bags are subject to search by the chaperones. Any teen with specific dietary needs should contact Mrs. Lietaert so we can make necessary preparations.

*No medication will be allowed unless the chaperones have been notified in advance. Medical release forms are required for all teens attending.

*Only those teens that register will be allowed to attend the Lock In. The deadline for registration is Tuesday, June 27, 2017. Registration forms, permission slips and medical release forms can be emailed to Mrs. Lietaert.

*No teens will be allowed to leave early before 12 am unless arrangements have been made in advance. If a special need should arise, parents must be contacted before a teen will be allowed to leave SJA. Parents must provide a phone number where they can be reached during the hours of the event in case of an emergency.

*Parents will be contacted if any teen acts in a manner deemed unacceptable by the chaperones. Chaperones and all participants are to be treated with respect.

*CELL PHONES SHOULD BE OFF ALL NIGHT! Participation in Lock-In events is expected – cell phones will be allowed for emergency use only. Teens should wear comfortable clothes and can bring pillows or blankets if they wish. Teens should not bring personal electronic devices.

*Treat all property and activities with respect. Teens and their parents are responsible for any damage caused.
