Time Allotted / 210 MinutesHazardous Haze – Start Exercise
During the exercise it will be important to remember and comply with the following statements:
· It is important NOT to jump ahead in the exercise scenario. In other words don’t focus on what might happen, focus on the Common Operating Picture that is presented during the exercise to make informed decisions.
· In any exercise a number of assumptions and artificialities may be necessary to complete play in the time allotted. During this exercise, the following apply:
1. The scenario is plausible, and events occur as they are presented.
2. There is no “hidden agenda”, nor any trick questions.
· Exercise Basics:
1. This exercise contains a total of 12 scenario and inject pages.
2. The exercise is self-paced and it should take approximately two hours to complete. However, based on the time spent on each scenario and inject the exercise may be completed in under two hours or it may take much longer than two hours to complete the exercise.
· Thoroughly review all scenario information and injects as they are presented during the exercise.
· After 2:00PM ET today each registered participant will need to log into the ONX System to complete their individual Hotwash to access their Certificate of Participation.

Hazardous Haze – Scenario – # 001 / Image of School Bus & Crash Site Map
Early in the evening on an unusually warm and humid September day, eighth grade students from Neptune Middle School are returning from their first field trip of the school year. The school bus in which they are riding on is traveling through the township along Memorial Drive as it makes its way back to school.
Meanwhile a delivery truck is traveling down the same street in the opposite direction.
Suddenly the truck driver grasps his chest in pain and slumps over the steering wheel. Within moments the truck collides with the school bus.
A motorist waiting at the stop sign on Eighth Avenue to turn left onto Memorial Drive witnesses the collision and uses his mobile phone to call 911.
An Emergency Communications Officer located at the Monmouth County 911 Communications Center answers the incoming call and begins to gather information from the caller.
The caller states that he has just seen a large truck cross the centerline of the road and strike a school bus on the left front side. The caller also states that it appears the accident occurred close to the intersection of Memorial Drive and Ninth Avenue.

Based on the information provided by the caller to the Emergency Communications Officer identify the response units that will be dispatched to the scene of the accident.

Hazardous Haze – Inject – # 003 / Image of Chemical Container
A cruiser from the Neptune Police Department is the first unit to arrive at the scene of the accident. As the officer steps out of the vehicle he notices that the school bus has collided with a 28 foot delivery truck belonging to ACME Chemical Company.
The side of the delivery truck has been torn open and its contents lay scattered across the road. It appears that the truck contains a wide variety of containers from 5 gallon buckets to 55 gallon drums to 50 pound bags all filled with various chemicals.
Seeing that the delivery truck’s contents are scattered across the road should the officer approach the accident scene? / Yes / No
Briefly describe the actions that would be taken by the officer at this point.

Within a few minutes the Fire Department and EMS arrive at the accident scene.
As several of the responders exit their vehicle they begin to smell the noticeable odor of ammonia in the air.
Briefly explain the on-scene actions that will need to be taken at this point.
Would the Monmouth County 911 Communications Center be contacted to dispatch additional resources to the scene of the accident? / Yes / No
If the above answer is yes. – Identify the additional response resources that would be dispatched by the communications center.
Knowing this is a potential HazMat response along with the fact that a school bus is involved in the accident, prepare a list of other agencies and organizations, in addition to the response resources listed above, in which the communications center would be responsible for contacting.

Hazardous Haze – Scenario – # 005 / Image of Injured Students
The school bus was carrying 22 students and the driver at the time of the accident.
Immediately following the accident four of the students that were not injured along with three students with minor injuries exited the bus. After leaving the bus they decided to wait just outside the bus for help to arrive until they noticed the strong smell of ammonia. They immediately moved up-wind from the accident site.
The remaining 15 students are too seriously injured to try to leave the bus and are yelling for someone to come help them.
The bus driver is pinned between her seat and the steering wheel and it appears that she is unconscious.
Several students have used their mobile phones to call 911 and their parents.
The driver of the delivery truck does not appear to have sustained any major injuries due to the accident. However he does not appear to be conscious.

At this point the Police and Fire Departments along with EMS are on-site; however the township’s HazMat Unit has not arrived.
Would On-Site Incident Command System (ICS) be established at this point? / Yes / No
If the above answer is yes. – Identify the individuals by name, title and agency that will be responsible for fulfilling the various ICS roles for this incident.
(Example: Incident Commander – John Harper – Lieutenant – Fire Department)
If the above answer is no. – Briefly explain why ICS would not be implemented.
The HazMat Unit has just arrived on scene.
Will the On-Site ICS roles be modified after the HazMat Unit arrives? / Yes / No
If the above answer is yes. – Briefly explain the ICS role changes that would be made.
If the above answer is no. – Briefly explain why there would not be changes to the ICS roles.

As the sun begins to set it remains rather warm for this time of year and there is a steady breeze from the east.
Based on current known conditions and the concept of a team approach to managing the incident develop an Incident Action Plan (IAP) that focuses on the INITIAL RESPONSE ACTIVITIES that need to be taken.

List the expendable supplies and / or equipment that will be needed to establish on-site communications.
Identify the various radio frequencies that would be utilized to support on-site communications.

Would the township’s Reverse 911 system be utilized to notify the public during this event? / Yes / No
Identify the individual by title that would be responsible for making the decision to use the township’s Reverse 911 system.
If the above answer is yes. – Briefly explain how the Reverse 911 system would be activated and describe the content of the message that would be broadcast via the system.
