By signing below,you are hereby authorizing Planet Safety LLC(PSLLC) to act on behalf of the USPPI to sign, endorse and process all documents for export control, Census Bureau reporting and U.S. Customs and Border Protection Purposes in connection with your shipment. The USPPI certifies that all information and documentation provided are hereby true and correct. False statements and fraudulent documents are subject to possible Civil and Criminal Fines and Penalties.

The above is a correct declaration, and should the shipment for any cause be refused or returned, I agree to pay any and all charges incurred.

Warehouse and property liability is limited to $500 per unit provided cargo is declared, ocean freight paid and cargo manifested on the Bill of Lading. Marine Insurance is offered and will only be in effect if requested and paid for at time of shipment. Premium rates provided upon request.

I certify that there areNO NON-ALLOWABLE commodity or HAZARDOUS MATERIALS being shipped in my vehicle or personal effects, such as (alcohol, spirits, liquids, flammable, explosives, aerosol cans, weapons, tobacco or tobacco products, etc.).

Shipping Charges do not include U.S. customs x-ray exams or U.S. customs inspections. If a U.S. customs inspection occurs, all charges are payable by the cargo owner / shipper. Cargo will not be released by customs until such charges are paid.

It is the full responsibility of the Cargo Owner to provide complete and proper documentation and information that is required in order for the shipment to take place within the laws and regulations as set forth by all applicable U.S. Governing agencies.

All Shipments are prepaid unless other arrangements have been made and agreed upon. Destination charges are not included in the U.S. charges provided, unless otherwise stated and paid for at time of shipment. Destination Port Charges, Unloading, Dray, handling charges, customs clearance, import duties and taxes when applicable are payable by the cargo owner at the destination arrival point.

*Marine Insurance premiums are based on 1.2% of the vehicle value. Minimum premium $180. Premiums are with a 1% Deductible policy or Minimim $250 deductible. To calculate Insurance Premium = Vehicle Value x 1.2% = $ Premium Amount – Example: $15,000 vehicle value x 1.2% = $180 premium. Deductible will be the minimum $250

Payment - Shipping charges are due upon receipt of invoice. Ocean Bills of Lading and your cargo will not be released until full payment is received. Late payments are subject to interest charges. Planet Safety will not accept claims for storage or demurrage charges caused by non-release of cargo.

All U.S. charges are due upon receipt of invoice. Any unpaid balance of the U.S. charges that have not been received by vessel arrival at the destination port will be subject to a 10% COD Fee if Planet Safety has to have the steamship line or destination agent collect charges on our behalf, there will be an additional 10% COD fee imposed.

Destination charges are not included in the U.S. Charges provided. Unloading, Dray, handling charges, customs clearances, import duties and taxes when applicable are payable by the cargo owner at the destination arrival point.

I hereby understand that all shipping charges provided prior to the actual shipment does not include any U.S. customs x-ray exams or U.S. customs inspections. If a U.S. customs inspection occurs, all charges are payable by the cargo owner / shipper and U.S. customs will not release cargo for exportation until such charges are paid.
