State of Washington Licensed Chemical Dependency Counselor

FLSA Status:Non-Exempt

Estimated Term: 7/13 thru 9/30/14

Hours:Minimum: 16 hours per week

Maximum: 20 hours per week

SUMMARY: The Shoalwater Bay Indian Tribe – Shoalwater Bay Wellness Center is soliciting for an Independent Contractor to provide chemical dependency assessments and/or appropriate transfer of care for those currently in treatment with Shoalwater Bay Wellness Center. Proficiency with TARGET data entry and computer usage. Ability to understand and utilize computer software programs. Ability to formulate and submit reports. Extensive experience dealing with substance abuse. Ability to provide extensive and intense counseling. Ability to manage time and tasks. Ability to accurately document client and project activities. Sensitivity to the comfort of client. Remains committed to provide current and up to date Chemical Dependency information to enhance programs effectiveness. Required: State of Washington Licensed Chemical Dependency Professional (CDP)

The Scope of Work includes, but is not limited to:

Provide chemical dependency assessments and/or appropriate transfer of care for those currently in treatment under the Shoalwater Bay Wellness Center IASAP/DOJ grant.

Document all patient contacts in the patients Chemical Dependency Record in a timely manner based off WAC.

Base decisions on information obtained through professional counseling knowledge and skills, current accepted standards of care and approved policies and procedures of the Shoalwater Bay Indian Tribe.

Requirements: Must maintain current, active, valid, unrestricted State of Washington Chemical Dependency Professional licensure. Must work cooperatively with the Health Director, other providers and staff, as well as the Grantor of the funding program. Must observe strict patient confidentiality and observe all requirements of HIPAA regulations. Proficiency with data entry and computer usage.
