Service User’s Safety/Risk Identification and Response

Purpose / To collaboratively identify and manage risk in order to provide the safest possible recovery and treatment context for clients, their family/whānau /carers and the wider community.
Scope / name of serviceemployees and other relevant service providers.
Contracts/agreements that include ‘assessment’ in the Tier 3 ‘Service Component Processes’ are required to identify and manage service user’s risks (in line with their overall service delivery model).

Definitions
Risk / The likelihood of an adverse event or outcome.
Risk factors / The particular mental health condition and the behaviour or circumstances that alone or in combination lead to an increased risk.
Risk screening / Initial brief check of risk such as risk alerts.
Risk assessment / An estimation of the likelihood of particular adverse events occurring under particular circumstances within a specified period of time.
Risk formulation / A process of summary and organisation of the risk data, and identification of the risk factors. It provides the information base for risk management
Risk management / Aims to minimize the likelihood of adverse events within the context of the overall treatment and recovery processes of an individual, to achieve the best possible outcome, and deliver safe, appropriate and effective support and interventions.
