Please be very cautious when you are filling out a form that requires your signature. When you sign it, the form becomes a legal document. You have agreed to everything above your signature.
In 2017 and also in April 2018, when I wrote the exemption updates for those two years, I mentioned the "Your Choice? Your Choice" section on the vaclib.org web site. In April 2018, I urged you to see that section, particularly pointing out a certain part of it. Now I am urging you to also read the following parts, in case you missed them.
The entire "Your Choice? Your Choice" section contains much important information. The page begins with the writers' disclaimer (please read it at and is followed by the section called "Your Choice? Your Choice."
Today I am highlighting these parts:

Warning About Physician's “Refusal to Vaccinate” Form Article title: "DO NOT SIGN the Refusal to Vaccinate Form!" [This warning includes any waiver/exemption form that could cause you liability problems, not just a physician's "Refusal to Vaccinate" form.]
What to do if you have signed the “Refusal to Vaccinate” Form Article title: "So, you’ve signed the Refusal to Vaccinate document?"[If you signed a form that contains statements with which you are uncomfortable, this applies to you. It tells why you need to use the Vaccination Notice.]
[Note: At the end of that section, the author states, "The Vaccination Notice with instructions can be found in the Overcoming Vaccine Mania Guide."The Overcoming Vaccine Mania Guide is a 2018 guide on the web site of Parents Against Mandatory Vaccines.

If you decide to sign a state authorized exemption form, you will not be using a Vaccination Notice. Be sure to read the section called "H. State Vaccination Exemptions" on pages 23 and 24 of the Overcoming Vaccine Mania - 2018 Guide. This section ends with these words:

Again, as long as moms and dads don’t challenge false or misleading statements on exemption/waiver forms, it will be assumed they agree with them. Should any employee of the school or public health department tell you that their forms cannot be altered, politely ask them for the number of the state statute that prohibits initialedclarifications. While these employees are bound by the rules of their employers, parents are not and should not hesitate to challenge their inane rules. If they don’t accept your appropriately modified waiver, move on to a Vaccination Notice.
