  1. Brown Expandable COLLEGE Folder (cf)
  2. This folder is for you to use to hold ALL college papers/information that you receive from colleges, especially the college you plan to attend for the next 2 to 4 years.
  3. Personal portfolios (pf) – briefly discuss contents in the pf.
  1. Senior Transcripts – preview, correct and return to counseling office; if have no corrections, then place in (cf)
  2. Permission Form for Release of Student Information (release to: Any school or agency requesting).

  2. Not sure what you want to do or where you want to go? Go to: do an undergraduate school sort again. Once you have narrowed your search of a school to go to, set up a VISIT. Visit at least 2 schools in order to compare.

  1. College Application process:
  1. Ask in office for an official copy of your transcript to be sent to the college(s) of your choice.
  2. Fill out application and write a check for the fee
  3. Common Application Process – (Filling a common application form)
  4. Verify information (check and then re-check to make sure you have all information the college wants)
  5. After 2 weeks: Call the college(s) admissions office to verify they have received all necessary information.
  6. Most Minnesota Schools offer a two week window in the fall to apply FREE online.

  1. School & rep. visits. (TOP 3 choices); Listen to Announcements/Check with Office
  1. Selective Service sign up, ( all males must register before their 18th birthday.
  1. Sports & Collegiate eligibility: If you are planning to participate in sports at the NAIA or NCAA level you need to send them your ACT score and a copy of your final transcript. The ACT Code’s are 9876 (NAIA) and 9999 (NCAA). In the spring remember to add the NAIA or NCAA Eligibility Center to your official transcript request from the CHS Counselor’s Office.
  1. ACCUPLACER Test
  2. Usually used by 2 year community and technical colleges.
  3. Is usually taken at the school you plan to attend but not always.
  4. Is offered at MN West the first Tuesday of each Month at 9:00 or by appointment
  5. NOTE: This is the test that colleges usually have you take to determine if THEY believe you need to take remedial classes. Remedial courses are ONLY a recommendation NOT a requirement from the college. They recommend them for a multitude of different reasons.
  1. ACT Test
  2. Usually used by 4 year schools for college entrance and academic scholarships.
  3. Test date in Canby this year is Saturday, December 10. Registration deadlinesare usually 5 to 6 weeks in advance. *ACT wants registration done via the web at IMPORTANT: You must provide a photo and your high school code when REGISTERING.
  4. Basic Registration Fee: (Varies there is an additional charge for taking the writing test. Around 35 to 50 dollars.)
  5. ACT 4-digit institutional codes are used to send institutions your ACT scores.

  18. The Financial Aide Office of the college you will be attending is a great resource for more information/scholarships/etc. They will be able to answer the majority of questions regarding financial aide.
  19. HOSECK FAMILY SCHOLARSHIP/SDSU - $1000 award and may be renewable if recipients maintain the necessary requirements. Students must fill out the standard scholarship application for SDSU and write Hoseck Family Qualifier next to your name on the application.
  1. FAFSA – Free Application for Federal Student Aide
  2. Must be filled out to qualify for ANY type of Federal Financial Aide.
  3. To fill out the FAFSA go to and follow the directions.
  4. May be submitted beginning in October of this year for next year. (Major change for this year.)
  5. Gather all necessary information before beginning to fillout.
  6. Check out the financial aid estimator:
  1. Financial Aid Night--Monday, October 10 from 3 to 4 pm and 6 to 7 pm in the HS Media Center. A one hour session will follow each to help parents and students set up a FAFSA ID.
