SENIOR CARE WORKER / TRUSTED ASSESSOR

Responsible to: Urgent Team Care Manager/Co-ordinator

Hours: 37 flexible hours per week over 7 days

Location: East Kent / West Kent

General Brief: Senior Carer Support workers (Trusted Assessors) will work as part of a small Urgent Response team, delivering rapid response care services for Carers in crisis, people who need support to stay in their own homes to avoid hospital admittance, or to support people to come home from hospital as soon as possible.

Post holders will work as part of a flexible rota to ensure sufficient capacity within the organisation to provide urgent/crisis support, whenever it is needed across a 24/7 period. In order to provide the appropriate level of care and support the senior care worker (Trusted Assessor) must also be able to carry out all the tasks set out in the Carer Support workers job description.

As a first responder the role of senior care worker (Trusted Assessor) will be able to make a rapid assessment of each urgent referral allocated to them, provide calm and practical, support to the individuals concerned, and to liaise with the Urgent Team Co-ordinator/Team Leader/Care Manager to co-ordinate and deliver a plan of care and support to best manage the crisis. The objectives of the intervention are to reduce stress, prevent hospital or residential care admission wherever possible, and to maintain the caring situation.

Principle Tasks:

Carers and People with Care Needs

v  To attend families and individuals with care needs, including dementia, under the direction of the Urgent Care Team Co-ordinator/Care Manager.

v  To assess the level of need, complete risk assessments and care plans with families, individuals, and relevant professionals as required, to determine the level of assistance to be provided directly by Crossroads Care

v  To assess Carers and people with care needs’ eligibility for a Crossroads Care service within our terms of reference as directed by the Urgent Care Team Co-ordinator/Team Leader/Care Manager

v  Under the direction of the Urgent Care Co-ordinator/Team Leader/Care Manager make referrals to other agencies where appropriate and with the permission of the family

v  Ensure that care plans are written, reviewed and accurate records maintained in client’s notes and office copies as required.

v  To carry out service reviews and service evaluations, raise any issues or concerns to the attention of the Care Manager in accordance with Safeguarding procedures and policy

v  To carry out Health and Safety Assessments in the home as required

v  Work with the care management team to ensure that all aspects of service provision comply with National Care Standards, CQC and Crossroads Care’s own quality standards – CROQUET

Staff Responsibilities

v  To attend all training, supervision and staff meetings as required

v  To attend case reviews and other related meetings as required

v  To mentor through shadowing, and support new team members as required.

v  To raise any issues or areas of concern with the Urgent Care Co-ordinator/Care Manager

Training

Trusted Assessors will be required to attend relevant training courses and staff development programmes to ensure that their own training needs are met, in addition to Crossroads Care’s carer support worker training.

General

v  To promote and represent Crossroads Care to Carers, professionals and the wider community

v  Provide written case studies and reports as required to ensure that the Senior Management Team is kept informed of all current activities

v  To undertake any other tasks as agreed to ensure the smooth running of the service.

v  To provide holiday and sick cover for other Seniors (Trusted Assessors), as required.

The post holder will be expected to use his/her initiative to work under minimum supervision and in close co-operation with team colleagues

Essential Person Specification

-  Own Car - Must be prepared to travel across East Kent or West Kent, as required.

-  Own Mobile Phone

-  Minimum GCSE or equivalent in Maths and English, Grade C or above

-  NVQ 3 in Health and Social Care

-  Certified in all mandatory care courses (moving & handling, MCA, DOLS, Medication)

-  Dementia trained and have experience in supporting or caring for people with known or suspected dementia

-  Excellent communication skills, resourceful and committed to supporting individual outcomes

-  Able to listen, understand and motivate people

-  Patient when under pressure and able to deal with emergency situations
