This report must be submitted to the SU no later than Monday 12th December, and all other relevant deadlines must also be met.Please ensure you are answering Yes to all of the checklist items and evidence provided, and note any further details, e.g. dates of upcoming events; information on meetings etc. PLEASE COMPLETE ALL SECTIONS OF THE FORM. If you haven’t achieved anything on the bronze, silver or gold targets, you can just write ‘No’ in the checklist column. Please note we will take into account areas from levels 1, 2 and 3 to work out your overall accreditation level even if one level hasn’t been fully completed.

Level 1: Bronze

Level 1 activities are worth 5 points each. You need a minimum of 20 points to be awarded a bronze allocation.

Level 2: Silver

Level 2 activities are worth 10 points each. You need a minimum of 60 points to be awarded a silver allocation.
