5.0 MANAGEMENT SUPERVISION

5.1 ScientiOrganizational Chart

5.1.1  HSE Organizational Chart

The TDI-BI HSE officer shall have the overall responsibility for achieving HSE objectives. The HSE officer shall communicate HSE issues to both management and vessel staff. The TDI-BI HSE officer shall be responsible for keeping abreast of HSE regulations and policies. The TDI-BI HSE officer shall ensure that the vessel HSE officer receives periodic training and will serve as a resource to that position. The TDI-BI HSE officer shall conduct twice yearly audits of the vessel. The vessel shall have a trained safety advisor on the crew at all times. The HSE advisor/officer shall ensure that all ship’s crew, scientific party and observers understand and observe HSE policies. The vessel HSE officer shall conduct routine safety meetings and inspections.


The following chart describes the roles of both ashore and vessel management with respect to the health and safety of the crew, visitors, contractors and scientist on our vessels.


5.1.2  The corporate structure for TDI-BI is as follows:

5.2 HSE Meeting Structure

5.2.1  Meeting Schedules/Frequency

TDI-BI Corporate Officers, the HSE Officer and designated key personnel shall meet at a minimum on a quarterly basis to review compliance with the HSE Plan. Safety meetings will be held on board all TDI-Brooks vessels weekly while conducting projects. It is the responsibility of on board HSE advisor or Party Chief to orient all new employees on all TDI-BI Safety Documents.

In addition, if required TDI-BI, will hold a pre-survey Management Kick-Off Meeting four to six weeks prior to each project. This meeting will discuss Company HSE requirements and TDI-BI HSE processes. This meeting is to be attended by TDI Corporate Officers, HSE Manager/Officer, Party Chief, HSE advisor, and Company representatives. Topics to discuss may include a review of HSE audits, hazard registers and risk assessments, and insure all permits and licenses are in place as required by the client.

Prior to beginning a new project, TDI-BI will participate in a pre-survey hazard identification and risk assessment. This meeting will be held at the request of the client and will develop specific preventive and mitigation measures for identified hazards. Preventive and mitigation measures will be implemented by TDI-BI prior to and during the course of project operations.

Also prior to the start of a project, a HSE meeting will be held between TDI-BI management, HSE representatives, company representatives and others necessary. At this meeting TDI-BI will provide a project specific HSE Plan for client review. At this meeting specific HSE issues involving the project specific HSE documents will be addressed.

Immediately before beginning the project a general HSE meeting will be held for TDI-BI employees, subcontractors and client representatives. This meeting will be mandatory for all individuals sailing on the vessel, with the exception of necessary watch personnel (who must be briefed at another time). Topics to be covered during this meeting include: HSE processes, advise personnel of HSE requirements and procedures, advise personnel of permit restrictions and/or government regulations, advise personnel of identified HSE hazards associated with project, mitigating measures and proper procedures to follow, advise personnel of their HSE responsibilities, review ERP, complete HSE training, implement Job Safety Analysis program and provide all HSE documentation. Moreover, weekly safety meetings will be held by the shipboard HSE advisor. Daily meetings, known as “toolbox meetings” will be held for each work shift (minimum 1 daily) while conducting ship’s operations. These daily meetings will be informal information meetings designed to discuss the days activities and potential health, safety and environment hazards/issues since the last toolbox meeting. Toolbox meetings are typically 5 to 15 minutes in length and will typically consist of a group on individual who work together. The following three groups will hold toolbox meetings: 1. bridge, 2. deck crew, 3. engine room crew, and 4. back deck crew. The recommend time for these meetings is at watch changes so that both watch crews can communicate issues during the concluding and upcoming watches.

5.2.2  Attendees/Minutes/Reporting

Meeting attendees will be logged and minutes taken and filed with the HSE Officer and in the corporate office of TDI-BI located in College Station, Texas. Copies of minutes will be furnished upon request.

5.2.3  Agenda and Content

Ongoing agenda items will include reviewing drill evaluations and MedEvac plans, safety inspections, safety compliance and suggestions, reviewing accident reports and informing employees of new safety procedures or techniques on crane usage, fire safety, first aid, etc.
