Secondary School Nomination Form
Secondary School Awards are open to Secondary School students who represent their school and are under the age of 19 and of Year 11 or above status as at 1 January 2017 (with the exception of students with a disability who are under the age of 21 at 1 January 2017). Exceptions may be given to younger students if the athlete has performed exceptionally within the given period).
TOI-OHOMAI INSTITUTE OF TECHNOLOGY SECONDARY SCHOOL SPORTSWOMAN OF THE YEAR
Open to any schoolgirl who has excelled in one or more sports.
· An outstanding sportsperson who has performed to an exceptional level in the
2016/17 specified year
· A sportsperson who has represented their school and region with distinction
· Has performed with a sense of Fair play
· Has made a contribution as an ambassador to their sport, school and the region

Open to any schoolboy who has excelled in one or more sports.

· A sportsperson who has represented their school and region with distinction
· Has performed with a sense of Fair play
· Has made a contribution as an ambassador to their sport, school and the region
BAY OFFICE PRODUCTS DEPOT BOYS’ TEAM OF THE YEAR
Open to boys’ teams that have performed to exceptional standards during the year.
· An outstanding Team who has performed to an exceptional level in the 2016/17 specified year
· A team who has represented their school with distinction
· The team performed with a sense of Fair play
· Has made a contribution as an ambassador to their sport, school and
the region
· Schools must select their top performing teams to a maximum of 4
nominations unless all teams have achieved National Division One titles
and if possible please rank these in order of achievement
BAY CARDIOLOGY GIRLS’ TEAM OF THE YEAR
Open to girls’ teams that have performed to exceptional standards during the year.
· An outstanding Team who has performed to an exceptional level in the 2016/17 specified year
· A team who has represented their school with distinction
· The team performed with a sense of Fair play
· Has made a contribution as an ambassador to their sport, school and the region
· Schools must select their top performing teams to a maximum of 4 nominations
unless all teams have achieved National Division One titles and if possible
please rank these in order of achievement
SECONDARY SCHOOL COACH OF THE YEAR
Open to any coach of a secondary school team or individual who has:
· Made an outstanding contribution in the area of coaching in the 2016/17 specified year
· Achieved results at the highest possible level within their sport
· Showed an innovative approach to coaching
· Made a contribution as an ambassador to the sport, school and
the region
· Performed with a sense of Fair play
AURECON BEST STUDENT CONTRIBUTION TO SPORT
Open to any student who has provided an outstanding service to sport
in their school.
· Open to any student who has provided an outstanding service to sport in
their school and community
SECONDARY SCHOOL SERVICE TO SPORT AWARDS
Open to any individual volunteer/sport administrator who has provided outstanding service to sport within secondary schools.
All nominations must be endorsed by the Principal of the nominee’s school.
