Second Annual Quality & Patient Safety Roadmap:

Eliminating Patient Harm Across the Board

July 19, 2014 to July 22, 2014 – San Diego, CA

Purpose:

This Quality & Patient Safety Roadmap is an event for charter members of the Symposium for Leaders in Healthcare Quality (SLHQ) and their patient and family advisors (PFA). The theme of the event is eliminating patient harm across the board. Roadmap will include plenary sessions, expert panels and interactive storyboard sessions that focus on engaging patients and families in eliminating patient harm.

Audience:

The 2014 Roadmap will bring together SLHQ members – quality and patient safety professionals – and patient and family advisors. All will participate in the plenary sessions, expert panels and interactive storyboard sessions together to encourage partnership and collaboration across disciplines in pursuit of eliminating patient harm. SLHQ charter members are encouraged to invite a PFA from their facility to attend both the Roadmap and Leadership Summit. Please view the HF/AHA Leadership Summit agenda here:

Objectives:PFA experts and national advisors will lead PFAs through a recap of the day’s events, as applicable to their PFA/hospital roles. The group will review strengths and opportunities of the day, and prepare for the following day’s activities.

Objective:Hospitals and their associated patient and/or family advisors will share their stories of eliminating harm across the board through storyboard presentations. Participants will engage with the hospital and patient quality teamsto gain feedback and coaching as applicable to their hospital role(s).
