SCIENCE SAFETY CONTRACT

The purpose of this contract is to make the student aware of his/her responsibility for laboratory safety. I WILL:

  1. Follow all instructions given by the teacher.
  2. Protect eyes, face, hands, and body when involved in science experiments.
  3. Carry out good housekeeping practices.
  4. Know the location of the first aid kit, eye wash, fire extinguisher, spill control materials, emergency shower and exits.
  5. Conduct myself in a responsible manner at all times.

I, ______, have read and agree to the following safety regulations set forth above and below in this contract. I will closely follow all oral and written lab instructions provided by my teacher and/or administration. I understand that I may lose my lab privileges if I endanger myself or others by not following the rules.

LABORATORY SAFETY RULES

I will:

1. Wear proper eye protection at all times in the laboratory. I will know the location and use of the eyewash equipment.

2. Confine or securely tie hair that reaches the shoulders. Hair is very flammable.

3. Noteat, drink, or chew gum in the lab. Dangerous chemicals may get in my mouth. I will never taste anything in the lab. No chemicals are to be taken from the lab.

4. Not enter the chemical storage are or laboratory without permission.

5. Not work in the laboratory or on an experiment without supervision.

6. Do only the experiments assigned and in the manner prescribed. Unauthorized experiments are prohibited.

7. Not attempt to duplicate any experiments at home.

8. Never engage in horseplay.

9. Wear footwear that completely covers the foot.

10. Avoid inhaling chemical fumes, avoid filling pipettes by mouth, and consider all chemicals dangerous.

11. Notify the teacher immediately in case of an accident.

12. Know the locations of fire extinguishers, fire blankets, eyewash, safety shower, first aid kit, spill control materials, and exits.

13. Notify the teacher immediately of any spills on skin or clothing.

14. Not use the sink as a waste basket, but dispose of materials into designated containers.

15. Not return any chemicals to the original container unless directed to do so.

16. Never point the end of a test tube at myself or at others.

17. Always add acid to water.

18. I will be responsible for the equipment I use. I will return it clean and dry. I expect to pay for equipment that I break.
