CLINICAL PLACEMENT AGREEMENT

Clinical placements are an essential and integrated part of the BSc (Hons) Diagnostic Radiography Programme.

In order to gain the necessary range of experience in clinical practice, students will be required to attend the clinical placementdeemed most appropriate by the Placements Officer.

Clinical placement is usually for 7 working hours per day, 4 days per week (maximum of 28 hours per week). There is some flexibility around the start and finish times which can be negotiated with the base hospital to which you are allocated.

The clinical placements are located across a wide geographical area and may require the student to relocate.

Please complete the pro forma below. It allows you to rank the clinical placements in order of your preference. You must rank all the placements.

Students in receipt of a bursary may claim back dual accommodation charges and placement travel expenses. Self-funding students (or those in receipt of an LEA grant) are unable to claim back these costs.

CLINICAL PLACEMENT PREFERENCES

In the grid below write a number between 1 and 8 for ALL of the clinical placement base hospitals in order of your preference. Your most preferred option should be given a 1 and the least preferred an8.

Use the web links to access the web sites for each base hospital. You will be able to find out information about each of the hospitals and how to get there. This information may help you to make a decision about which base hospital you would prefer. Please note that some NHS Trusts have more than one site. You will be expected to rotate between these other sites.

Newcastle upon Tyne NHS Foundation Trust (
It consists of three main sites: the Freeman Hospital, Royal Victoria Infirmary, and Newcastle General Hospital.
Gateshead Health NHS foundation Trust Queen Elizabeth Hospital,
It consists of one three main sites: the Queen Elizabeth Hospital, Bensham Hospital, and Blaydon Primary Care Centre.
City Hospitals Sunderland NHS Foundation Trust (
It consists of three main sites: The Sunderland Royal Hospital, and other primary care sites in the area.
County Durham and Darlington NHS Foundation Trust (
University Hospital North Durham
It consists the University Hospital of North Durham and there are also two community hospitals based at Shotley bridge and Chester-le-street.
County Durham and Darlington NHS Foundation Trust (
Darlington Memorial Hospital and Bishop Auckland General Hospital – this involves travel between these two sites.
North Tees and Hartlepool NHS Trust (
It consists of three main sites: University Hospital of North Tees, University Hospital of Hartlepool, and Peterlee Community Hospital.
South Tees Hospitals NHS Trust (
The James Cook University Hospital in Middlesbrough. There are also some additional satellite units in Redcar, Brotton, and Guisbrough.
South Tees Hospitals NHS Trust (
Friarage Hospital in Northallerton. There are also some additional satellite units in Richmond.

I agree to go to any placement allocated to me by the Clinical Coordinator. I understand that I may be required to relocate for these placements. If I am not in receipt of a bursary, I understand that I will be unable to claim back placement travel expenses.
