School Based Eye Care Program

We do COMPLETE Vision Exams, not vision screenings !!!

Dear Parent / Guardian,

Vision services (eye exam) will be available to your child, during school hours. These services include: visual exam, visual diagnostics, lens prescription, eyeglass fitting, eyeglass dispensing, visual problems identified, “lazy eye” (amblyopia) identification. The school optometrist will decide what services are necessary and appropriate for your child. Some procedures may require the use of “eye drops” to dilate the child’s eye (s). The Mobile Vision Team has just completed its fifth year conducting examinations in schools.

There is no cost to you for this vision program, but it’s not “free”. Insurance coverage helps cover our costs. If your child has vision insurance, please complete the necessary information on the permission slip. We will SUBMIT A CLAIM to your insurance company, including Medicaid and private insurance. There is no copay! After our eye examination, there is no need to schedule an additional eye exam with another eye doctor. We do a complete eye exam.

If you would like your child to be part of the School-Based Eye Care Program at school, you will need to COMPLETE AND SIGN the attached permission slip and return it to the school nurse. We will come to each school annually. No eye examination will be done without a signed permission slip on file at school!

·  After obtaining your permission, we will schedule your child with the school optometrist.

·  You will be notified of all completed treatments through a letter sent home with your child.

·  If your child has had recent vision care, please note this on the permission slip, as we do not want to duplicate services.

·  Please understand that some conditions may be too complicated to do at school. It may be necessary for your child to be seen and referred to a specialist. We will assist you with identifying any appropriate specialist.
