TERMS AND CONDITIONS
01. National/International manufacturing firms or their Authorized Distributors will be eligible for participating in the Tender.
02. The tender should be sent to the Superintendent, SCB Medical College Hospital, Cuttack- 753007 (Odisha) through Registered post/ Speed post only and should reach on or before 03.11.2015 up to 1.00 P.M. Tenders received beyond the scheduled date and time will be rejected.
03. The tenderers should furnish E.M.D. of Rs. 20,000/- ( Rupees Twenty Thousand) only in shape of fixed deposit / DD in the Nationalized Bank/ Post office duly pledged in favour of the Swasthya Bikash Samiti, SCB Medical College Hospital, Cuttack.
04. The tenderers should furnish self attested Xerox copy of up to date Drug License/ VAT clearance certificate in the form of VAT-612/ Income Tax, Pan Card along with their tender.
05. The tenderers should submit the copy of money receipt in token of purchase of tender paper alongwith the tender.
06. The Authorized Distributor should furnish the Authorization letter in original received from the competent authority of the manufacturing firm addressed to the Superintendent, SCB Medical College Hospital, Cuttack alongwith the tender.
07. Request for adjustment of E.M.D. submitted against any previous tender will not be accepted.
08. No advance payment against the execution of purchase order is admissible.
09. The price of the items should be excluding of all taxes.
10. The price should be F.O.R. destination at the Central Store of SCB Medical College Hospital, Cuttack.
11. Reagent/chemical/kit supplied must have expiry date at least 8 (Eight) months from the date of supply.
12. If any information or documents furnished by the tenderers are found to be misleading or incorrect at any stage, the tender will be cancelled and will be blacklisted for a period of five years in the SCB MCH, Cuttack.

13. All documents i.e. money receipt, EMD, PAN card, Drug license, authorization letter, VAT clearance certificate, terms & conditions of the tenderers should be sealed in one envelop superscribed with “Technical bid” and price list sealed in one envelop superscribed “Price bid”. Both the envelops should be sealed in another envelop superscribed with “Tender for laboratory reagents/ chemicals/ Laboratory wares (Users Fund).”
14. The technical bid envelop will be opened on 04.11.2015 at 4.00 P.M. in presence of the tenderers or their authorized representative, who should be present in the office chamber of the Superintendent, SCB Medical College Hospital, Cuttack. If the tenderers/authorized representatives fails to turn up that will not be a bar on the authority for opening of the tenders.
15. The date and time of opening of price bid will be intimated to the qualifying tenderers in due course.
16. The undersigned/ Committee is not bound to accept the lowest price considering the technical aspect and reserves the full right to reject/cancel any or all the tender without assigning any reasons thereof.
17. The tenderers may download the documents directly from the WEBSITE available at http://www.scbmch.nic.in & demand draft of Rs.1,000/- (One thousand) only in favour of Swasthya Bikash Samiti, SCB Medical College & Hospital, Cuttack, payable at Cuttack, which will be enclosed alongwith technical bid and the tenderers should superscribed ‘DOWNLOADED FROM THE WEBSITE’ on the left corner of the outer envelope.
