Print and fill out the Vision/Hearing Screening (SAT Form LCPS VHS)
To Parents:
Print Family Letter
Print Case History for Initial Academic Referral (SAT Form Academic CH)
To Student:
Print Student Survey (SAT Form SSUR) and help student complete it, if they need help.
From Administrator: Get a Cognos Report

Completed SAT Referral Packet with attachments, if applicable:
·  Student’s Academic Improvement Plan (make sure all interventions have been documented)
·  Student’s Home Language Survey (found in cumulative folder)
·  Student Work samples, short-term assessments
·  Discovery scores print out
·  Behavioral or disciplinary referrals
·  Medical / health information
·  Social worker or counselor information
Diagnostician / Turn BASC and LDES to Diagnostician for scoring. Wait for report before meeting.
SAT Lead / Enter information into Synergy SE (SAT Form Initial SAT Entry)
SAT Lead / Send out Invitation to SAT Meeting (SAT Form ITM) to parents
SAT / At Meeting:
·  Team will review SAT Referral Packet, documentation and COGNOS report.
·  Review results of BASC Screener and LDES Screener.
The results of the LDES coordinate with the Learning Intervention Manual from
Hawthorne.
If the results of the BASC are elevated, possibly refer to mental health team.
Is speech an issue? See SAT Speech Referral Checklist
Is language an issue?
If so, please consult with building SLP to prescribe interventions for possible language issues.
If the applied interventions are ineffective, consult with SLP to determine if a Pre-referral Speech and Language Checklist (SLP will provide from Hawthorn) are needed. SAT plan will prescribe next cycle to include interventions from SLP recommendation.
If interventions have been applied and don’t work, complete a Request for Speech Screening (SAT Form SLS in Appendix) and give to Speech and Language Pathologist. Send Parent Consent to Screen (SAT Form PTC) to parents.
SLP will give SAT a SAT Follow-up Form (SAT Form Diag)
·  Complete Intervention Tracking Form (SAT Form ITF) by prescribing interventions, recording minutes and obtaining signatures.
·  Is an observation needed? If so, Print and fill out Parent Consent for Screening/ (SAT Form PC1). Obtain parent signature. Print Student Observation (SAT Form SO) and give to administrator after parent permission has been given.
·  Schedule next meeting to revisit prescribed interventions and analyze success.
After Meeting:
·  Put copy of Intervention Tracking Form (SAT Form ITF) in a yellow folder in Cum.
·  Give copies to staff that interact with student.
Referring Party
SAT / At next meeting:
Referring party will provide updated student data (observation, current CFA’s, Discovery, etc.)
SAT will determine whether interventions should be continued or if new interventions should be prescribed.
