AFTER INITIAL FILING: NOTICE:

I. Human Services Agency (HSA)

Mail copies of the following documents to: (Direct to relevant contact person at HASA, refer to the Addendum to Guardianship Manual as of May, 2016) San Francisco Human Services Agency, P.O. Box 7988, San Francisco, CA 94120-7988

SF Probate “Who’s Who Sheet”[including info on language preference of ward/guardian]

GC-020: Notice of Guardianship Hearing and Special Immigrant Juvenile Findings Hearing, Attachment to Notice of Hearing, and Proof of Service

GC-210/210(P): Copy of filed Petition

GC-248: Duties of Guardian

FL-105/GC-120: Declaration under UCCJEA

PGF-1: Declaration(s) of Proposed Guardian(s)

GC-212: Confidential Guardianship Screening Form(s)

  • if the proposed guardian or filing attorney has not heard from a court investigator regarding a home visit, follow up with the court investigator’s office to make sure they received the above paperwork;

For non-relative guardianships: mail a copy of the above packet to: State Department of Social Services, Director of Social Services, 744 P Street, M.S. 19-31F, Sacramento, CA 95814. A social worker from the Human Services Agency will conduct the background check and home visit.

II. Petitioner/ Client and Proposed Guardian

Petitioner/Client and Proposed Guardian: Provide copies of documents (provide the same documents as listed above for Agency), plus additional documents:

GC-220: Petition for Special Immigrant Juvenile Findings

III. Grandparents and Minor Siblings:

Grandparents and Minor Siblings –Mail a separate envelope containing a notice packet, which includes the following documents, to: (1) each biological grandparent and (2) each sibling over 12 (for siblings under 12, do not need to send separate notice if the adult they live with receives notice):

  • Cover letter explaining guardianship and special immigrant juvenile process (see sample in LSC sample forms)

GC-020: Notice of Guardianship Hearing and Special Immigrant Juvenile Findings Hearing , Attachment to Notice of Hearing, and Proof of Service

GC-210/210(P): Copy of filed Petition, including the following attachments:

GC-210(CA): Child Information Attachment

GC-211: Nomination of Guardian OR Waiver of Notice and Consent

GC-220: Petition for Special Immigrant Juvenile Findings

Minor’s Declaration in Support of Petition for SIJ Findings

IV. Biological Parents

Biological Parents –Mail an envelope containing a notice packet, which includes the following documents, to each biological parent:

Cover letter explaining guardianship and special immigrant juvenile process (see sample in LSC sample forms)

GC-020: Notice of Guardianship Hearing and Special Immigrant Juvenile Findings Hearing, Attachment to Notice of Hearing, and Proof of Service

GC-210/210(P): Copy of filed Petition

GC-210(CA): Child Information Attachment

GC-211: Nomination of Guardian OR Waiver of Notice and Consent

POS-015: Notice and Acknowledgment of Receipt (Biological parents can sign this form in lieu of Consent Form (GC-211))

GC-220: Petition for Special Immigrant Juvenile Findings

AFTER INITIAL FILING: BEFORE HEARING

File the below forms before the hearing (ideally two (2) weeks before the hearing):

GC-020 or GC-020(P)-Notice of Hearing cover page plus completed page 2 of GC-020 or CG-020(P) if personally served any party (only file if you have provided notice to additional parties not initially included on this page at initial filing)

GC-211: Any Consent of Proposed Guardian, Nomination of Guardian, and Consent to

Appointment of Guardian and Waiver of Notice forms received after the initial

filing.

POS-015: Notice and Acknowledgment of Receipt [Biological parents can sign this form in

lieu of Consent Form (GC-211)]

  • Guardianship Order and SIJS Findings
