Instructions

  1. Complete all information in ink and print legibly – pencil is not acceptable.If completing this form on a computer,an original ink signature is required.

  2. Refer to the payroll calendar under

“Beginning Date” – first day of pay period

“Cut-Off Date” – last day of pay period

  • Complete in format of month/day/year
  1. Complete Employee’sRegular Work Hours Per Day
  1. Write in the dates for the week in the Week box
  • SAISD’s work week is Monday through Sunday
  • Use the format: month/date - - Example: 3/29 - 4/4
  1. On the Hours Worked line
  2. Report all hours worked, including overtime
  3. A duty free lunch of 30 minutes or more is not counted toward hours worked
  • Record all time in decimals
  • Round time to the nearest quarter hour
  • Example: 8:05 a.m. is rounded to 8:00 a.m.
  • Example: 5:25 p.m. is rounded to 5:30 p.m.
  • Decimal Chart:

15 minutes = quarter hour= .25

30 minutes = half hour= .5

45 minutes = three quarter hour= .75

on the hour = whole hour= 1.0

  1. If absent from work – insert the hours and an absence code on the Leave Hours/Absence Code row under the day in which the leave occurred, to reflect the appropriate reason.
  • Absence codes are listed at the bottom of the form
  • Example: 8 – P (this example reflects 8 hours of personal leave)
  • Example: 4 – C (this examples reflects 4 hours of comp time off)
  1. An Absence from Duty Report is required for all absences
  • Including but not limited to: personal leave, sick leave, death in family, vacation leave, staff development leave, leave without pay, jury duty or subpoena

  1. Total Hours Column – Add and record hours worked for the week. Add and record hours of leave taken for the week.
  • If Total Hours (hours worked + leave hours) exceed regular hours, complete the reverse side of the time report
  • Reminder: Record all time in decimals (as stated in #6)

  • Signature certifies that the time report is an accurate record of the actual hours worked.
  • Falsification of this record could result in termination
  • Each employee is responsible for his/her time report

  1. Time Reports must be completed correctly and submitted timely or it may result in delay of paycheck. Refer to the campus or department administrator for dates that time reports are due in the campus or department office.

Overtime and Compensatory Time Procedures and Policy (DEA LOCAL):

Non-exempt employees who work more than 40 hours in a workweek of seven consecutive days are entitled to overtime compensation for time worked over 40 hours. Non-exempt employees shall be compensated for overtime in accordance with federal regulations by either time and one-half their regular hourly rate of pay or time and one-half earned in compensatory time off.

All overtime worked must be approved by the employee’s supervisor in advance of the overtime worked. Employees who work unauthorized overtime may be subject to disciplinary action. The supervisor and the employee must understand how the employee will be compensated for the overtime (compensatory time off or pay) prior to the employee working the overtime hours.

Compensatory time earned by non-exempt employees may not accumulate beyond a maximum of 60 hours. Compensatory time shall be used within the duty year in which it is earned. Use of compensatory time may be at the employee’s request or as determined by the employee’s supervisor to protect the district’s schedules and activities.
