RIVER/TEL:DSN 342-8052/TEL:301-342-8052/ EMAIL:// GENTEXT/REMARKS/1. This is a coordinated CNAF/USMC DCA message that supplements references a, b and c. It provides revised guidance for initial reporting of physiological events (PE) involving USN and USMC aircrew or aircraft.

2. Effective 15 January 2018, PE initial notification reporting will be conducted via email, and the initial notification via WESS is no longer required. Commanding officers shall send PE initial notification email within four hours of learning of a suspected PE for the following aircraft:

a. F/A-18 A-F or EA-18G TMS: PE initial reporting requirement will be sent to ?hornet_pe_notification(at)navy.mil?.

b. T-45: PE initial reporting requirement will be sent to ?t-45_pe_notification(at)navy.mil?.

c. T-6, EA-6B, AV-8B, F-5, F-16 or F-35: PE initial reporting requirement will be sent to ?sdni_cnap_tacair(at)navy.mil?.

d. F-35 additional requirement: within 24-hours, PE initial reporting should include an action request (AR) to the F-35 OPS center at Fort Worth, TX. This action will notify the joint program office (JPO) PE action team.

3. A physiological event (PE) is a specific subset of physiological episode (PHYSEP) where one or more aircrew exhibits physiological symptoms coinciding with or following an aircraft systems malfunction (ECS, OBOGS, or cabin pressurization malfunction) or aircrew life support systems. A system malfunction of these types with no associated aircrew symptoms is not a PE.

4. Initial email notification information requirements: the email will contain the mandatory reporting requirements contained in PE initial notification email templates and will specifically address the following critical information requirements:

a. Brief narrative of PE event (5w?s).

b. Was an aircrew member treated at a hyperbaric chamber?

c. Was the landing impaired?

d. Was there a foreign national aircrew member involved?

5. For USMC squadrons/units: use respective Marine aircraft wing flash report narrative/summary for the PE initial report email.

At a minimum, flash report narrative should include critical information requirements listed in paragraph 4. PE initial report email will be sent after flash report is submitted and shall include MAG and MAW leadership as addressees in addition to the distribution list mandated in paragraph 2.

6. Existing reporting requirements still apply: submission timelines for part A form (24 hours), part B form (10 days) and part C form (48 hours post medical treatment) remain unchanged. If a PE is categorized as a HAZREP or as a class C/D mishap, a HAZREP/SIR must be submitted in accordance with reference d.

7. The following pe resources are available at

a. PE initial report email templates for all affected TMS b. Current part A, B and C forms for PE events c. PE rapid response team (PERRT) SOP 8. This reporting requirement shall be incorporated into type-wing standard operating procedures and pre-mishap plans and binders.

Reporting agencies and squadrons can also access this information via an iphone application to simplify and standardize all mishap, incident and casualty response reporting at cnafcoguide. Included in the aviation module of the application is an interactive checklist for responding to physiological events.

The application is password protected and to receive login credentials, please email ?cnap_premishap(at)navy.mil? from your NMCI account. The android OS version of the application is in development.

9. Feedback on the event from the PE action team and Naval Safety Center will be provided at the earliest possible date after the event to all TMS squadrons, including administrative and operational chains of command. This will ensure information is available to all aircrew and maintenance personnel and to inform future decisions/ events.

10. Attacking the PE problem is Naval Aviation?s number one safety priority. This additional mandatory reporting requirement ensures the critical and timely information flow to better support the warfighter.
