______1) Is a member in good standing in a United Methodist Church.

______2) IsaCertified Lay Servant or equivalent, as defined bythe

CLM Oversight Committee of the Board of Lay Ministry. Provides a copy of current Lay Servant certificate to DCOM.

______3) Previously has discussedintentions to pursue CLM status

withone’s own pastor, who must submit recommendation in writing to DCOM.

______4) Sends documentation of approval from one’s SPRC to DCOM.

______5) DCOM receives affirmative vote by charge conference from

home church recommending entry as a CLM candidate.

______6) Submits the personal data inventory to the DCOM.

______7) Submitspermission form for background, credit, and other

checks to DCOM.

______8) Submits application to DCOM Registrar with supporting

documentation for the Certified Lay Ministry process .

______9) DCOM submits and receives reports from background checks

in the following areas: credit check, criminal record check, child abuse history, other ______. Cost to candidate is $35, payable upon application. All reports must be current within 5 years and at present address.

______10) Completes psychological assessment as defined by DCOM.

______11) Is in contact with the District Superintendent (DS) about the

candidate’s path, goals for service, and the assignment of a mentoring pastor or counselor.

______12) Candidate must demonstrate understanding of the

history, polity, doctrine, worship and liturgy of The United Methodist Church through service through one’s home church/SPRC/pastor.

______13) DCOM reviews data collected thus far, meets with the

candidate, and approves or denies candidacy.

______14) Obtains and begins Module I coursework. (ISBN #978-0-

88177-551-8 directly available from The Upper Room)

______15) Coordinates with DS to create a Mutual Ministry Team

Consisting of members of his/her local church. Submits a

list of members to the DCOM.

______16) Completes Module I.

______17) Begins Module II coursework.

______18) Completes Module II.

______19) Candidate decides whether to continue with CLM process or

opts for Local Licensed Pastor process (WHICH MEANS DISENGAGING FROM CLM PROCESS. )

______22) BeginsModule IV coursework.

______23) CompletesModule IV.

______24) Appears before the DCOM for review and approval.

______25) Uponthe candidate’s completion ofa successful interview,

the DCOM grantsCLM certification status, that serves as eligibility for assignment to a specific ministry by the DS. Notification to candidate must be in writing. (Certificate comes from Director of Mission and Ministries.)

Re-Certification

Once certified as a CLM, the individual is required to apply for re-certification annually by the DCOMafter successful completion of the following:

______1) A continuing education event (at least equivalent to an

Advanced Lay Servant Course – or material with prior approval by the DS or DCOM Chair.)

______2) Signed affirmation form received from CLM’s charge

conference . Form of continuance for CLM and that assignment have been signed by DCOM and DS and status remains “Certified Assigned.”

______3) Appearance before DCOM for interview and recommendation

vote.

______4) Recommendation of the Equipping Pastor.

______5) Recommendation of the DS.

______6) Renewal/updating of the covenant agreement if assigned.

______7) If CLM’s assignment is discontinued, conference status

becomes “Certified Unassigned.” If CLM remains unassigned for a period of two years, certification is discontinued.
