The Occupational therapist will need to spend an ……………..additional hours spread throughout the year to include carrying out admin/ non-therapy tasks such as attending meetings, note keeping, report writing, liaison with teachers, parents and other relevant professionals, school staff training.

The type and frequency of Therapy intervention will be reviewed annually as a minimum but may be amended to address any new identified specific therapy needs following discussion and agreement with the child’s named therapists.

More than one activity may take place within one visit / clinic appointment.

Educational Needs:

(Name) will require 1:1 adult additional support in school in order that he/ she can fully access the curriculum. This will include:

·  Implement a standing programme for up to1 hour every day.

·  Complete a fine / gross / sensory motor skills programme for 30 minutes per day.

·  Propel………..in their buggy / wheelchair around the school when required in order that ……….can access school buildings and outside play areas.

·  Use of equipment (seating, standing frame, side lying board) to meet……….postural needs.

·  Availability of education / support staff for relevant training by OT/PT staff relating to ………..physical management.

·  Assistance with all self care skills to include – daily toileting regime

·  Assistance on outings and school trips to ensure full participation in the event.

·  Assistance in the event of an emergency evacuation.

Environment:

·  A Risk assessment of moving and handling for all transfers should be carried out by Education and provision of equipment as indicated in the risk assessment.

·  An environment where staff have knowledge of physical disability which includes a physical management approach as an integral part of the day.

·  Time allowed for changes of position and use of equipment throughout the school day.

·  Suitable environment to accommodate and store equipment and safe use of the equipment.

Equipment Needs:

NHS THERAPY INPUT MAY VARY ACCORDING TO SCHOOL PLACEMENT AND SERVICE RESOURCES AVAILABLE.
