Dear Parent/Guardian;

We are pleased to be able to offer the Poley Mountain After-School Ski Program to RVMS students again this year. Although we would like to be able to offer this to all interested students, due to busing restrictions and Poley Mountain’s availability, we have limited availability.

We are requesting that all students put down at least a $25 deposit to have their name added to our ski Club list. This can be done beginning, Friday, November 18th. This deposit will be deducted from your total due below.

Please review the information below very carefully and complete both pages. Payments should be made with a $25 deposit immediately with the remaining balance due before Christmas. You may pay in two installments with post-dated cheques but all payments must be made no later than Dec 21st, 2016.

We will be using a full sized tour bus with washroom and storage below for ski equipment (Optimum Ride).

Please note:

1.  The program runs for six weeks on Thursday evenings beginning Jan 12th, 2016 (Jan 12, 19, 26 and Feb 2, 9 and 16). Students must commit to participating all six weeks.

2.  If Poley Mountain is closed on one of these dates, they will add another Thursday evening (Feb 23). If schools are closed or if the school makes a decision not to travel due to weather and Poley Mountain is open we lose our lesson for that week. We will make every effort to re-schedule but cannot guarantee this.

3.  The bus will leave our school at approximately 2:45 and return at approximately 9:45.

4.  The program includes a ski lesson. This lesson is geared to the skill level of the student and is mandatory. Students participating in this program cannot opt out of the lesson.

5.  Students must wear a helmet.

6.  Students may choose to ski or snowboard but snowblades are not allowed in this program.

7.  As this is a school sponsored event, school rules apply and school staff will be responsible for supervision of the students.

To be eligible for ski club you must:

1.  Bring a $25 deposit to Ms. McGrath in the school office as soon as possible.

2.  Fill out both forms attached (the last 2 pages) and send back to the school as soon as possible.

3.  Make remaining payment in full no later than Dec 21st, 2016.

The inherent risks of this activity may result in accidents arising from the nature of the activity and may occur without any fault on either the part of the student or Anglophone School District South or the employees of the facility where the activity is taking place.

By participating in this activity or allowing your child to participate in the activity, you are accepting the risk that you or your child may be injured. The chance of an injury occurring can be reduced by carefully following instructions at all times while engaged in the activity. If you choose to participate in the activity or allow your child to participate in the aforementioned activity on the date(s) mentioned, you must understand that you will bear the responsibility for any injury that might occur.

Anglophone School District South does not provide any accidental death, disability, dismemberment or medical expenses insurance on behalf of the students participating in these activities. Student Accident Insurance is recommended.

RIVER VALLEY MIDDLE SCHOOL is arranging a trip for students to go to Poley Mountain Resorts Ltd.
to Ski or Snowboard. This signed form is required for all students who wish to participate in this outdoor recreation and snow
sport education program. It should be understood that the purpose of this excursion is educational. Lessons are mandatory.
INHERENT RISK
Skiing and Snowboarding are a sport with physical demands and inherent risks which are beyond the control of Poley Mountain
Resorts Ltd., and River Valley Middle School. Falls, collisions and other incidents may occur which result in
serious injury or death. Participants must realize the inherent risks of the sport. Following the rules and procedures can
reduce the risk of injury. Failure to follow the rules will result in Students losing their lift ticket and future resort privileges.
EQUIPMENT
Poley Mountain Resorts Ltd provides rental equipment. Bindings on equipment reduce the risk of injury when falling. They
will not release under all circumstances and they do not guarantee safety in all cases. Parents must accept responsibility
for equipment that is lost or damaged (other than reasonable wear and tear.)
All participants must wear an appropriate Snow Sport Helmet for School Excursions at Poley Mountain Resorts Ltd. and
may not, under any circumstances, Ski or Snowboard on a trail marked higher than a blue trail. All Mini Terrain Parks,
Terrain Park and Terrain Park Features are off limits at all times to School Programs.
ALPINE RESPONSIBILITY CODE
The Atlantic Ski Areas Association has produced a Responsibility Code which Poley Mountain Resorts Ltd requires that you
know and obey. Skiers and Snowboarders must always be in control and be able to stop and change direction to avoid
collisions with people or objects. Poley Mountain Resorts Ltd. may revoke ski tickets for violation of the code or other
unacceptable conduct.
