IBM CORPORATION is awarded a contract for IBM CUSTOM SERVICES for a One (1) year, fifteen (15) day term from 12/16/99 to 12/31/00. This contract has an option to extend the contract for four (4) additional one-year terms upon mutual agreement.
Contract #0000000262 shall be in accordance with Attachment 1, Custom Services Agreement executed 12/16/99 between IBM and City of Seattle and Attachment 2, IBM Change Authorization for Project Support Services/Project Change Request dated executed 2/16/00. Contract #0000000262 is a contract for the specific use by the City of Seattle’s DEPARTMENT OF INFORMATION TECHNOLOGY to support the City in it’s administration of IBM systems located at the City of Seattle Computer Center, 220 Third Avenue S., Seattle, WA.

INVOICE AND PAYMENT:
IBM will submit monthly invoices to the City for work provided in the previous month.
Payment will be payable upon receipt.

INSURANCE:
IBM shall maintain, for the term of this Agreement, Commercial General Liability Insurance, Business Automobile Liability Insurance and Professional Liability/Errors and Omissions Insurance per Attachment 3 – Insurance Requirements. (IBM letter of December 16, 1999 Albert Traxinger/R.David Parker).
