Section E Outcomes
Set out outcomes which need to be achieved by the end of the key stage to enable the child or young person to make progress and support readiness for the next phase of education or transition to adulthood. Outcomes must be SMART(Specific, Measurable, Achievable, Relevant and Time Linked). School SEN Support Plans will map out the termly and/or annual targets to identify steps towards achieving the outcomes for the end of phase. Progress with these targets should be revised as part of the schools cycle of assess, plan, do and review.

Arrangements for review of the EHC Plan
For children and young people over 5 years a review of the EHC Plan must be completed within 12 months of the date of the first issue of the Plan and thereafter 12 months from the last review.
For children under 5 years a review of the EHC Plan must be completed between 3 and 6 months after the date of the first issue of the Plan and thereafter every 6 months.
