QUALIFICATION SUMMARY STATEMENT (ATTACHMENT 1 TORFQ)

General Information:The Offeror shall provide a summary of its competencies, experience, accomplishments and failures. Include with this summary statement contact information for each firm included on the Project Team. The submittal will be evaluated for scoring under this category based on the following information:

  • Overall Qualifications: Provide a brief description of the overall qualifications of the project team proposed to execute the engineering services for the Owner’s Representative. Explain how these qualifications will provide a value for the GMWWTP project.
  • Applicable Experience: The Offeror shall describe its technical experience as applicable to the GMWWTP project including but not limited to, alternative project delivery methods, domestic wastewater treatment, and landfill leachate treatment processes.
  • Project Team Composition: Identify all project team member firms including the firm name, business address, telephone number, and primary key contact. Describe the nature of activities each firm will perform with respect to the scope of work for this Project.
  • References: The Offeror shall provide no more than five (5) general letters of reference (per firm included in the project team) regarding similar projects performed within the last ten (10) years that encompassed engineering services similar in nature, scope, and budget to the Project. Letters of reference are not included in the 15-page maximum for Attachment 1.

Resume(s):The Offeror MUST attach a resume for each person identified on the key personnel list. Each resume shall not exceed two (2) pages in length. Resumes are not included in the 15-page page limit for Attachment 2.

Instructions for Narrative and Organizational Chart:The narrative section of Attachment 2 should be single-spaced in 12-point font on 8 ½ x 11 sheets with 1-inch margins. The organizational chart shall be provided on one or more 8 ½ x 11 sheets with 1-inch margins. The maximum number of pages for the narrative and the organizational chart combined may not exceed 15 pages.

General Information:The Offeror shall identify by name and summarize the qualifications of the personnel that may be assigned to the work described in Section 1 of the RFQ. The Offerorshall also identify the specialized skills and disciplines that will be required to complete the work.

The submittal will be evaluated for scoring under this category based on the information listed below.A key factor for successful implementation of the Project is ensuring qualified and dedicated staff are assigned for the duration of the Project. The proposal should address the following items:

  • Organizational Chart: An organization chart identifying key Offeror’s project team members illustrating their corresponding role with this Project. For each member listed in the organizational chart, provide a brief summary regarding the adequacy and appropriateness of their credentials and capabilities, expected project assignments, and the extent of their project participation, years of experience, and the relevance of their prior experience regarding alternative delivery methods, wastewater treatment, landfill leachate treatment processes, and expedited scheduling.
  • Personnel Commitment: The Offeror should demonstrate the commitment of its management staff, key staff and other full-time personnel throughout the life of the Project. The date of availability, the level of commitment and office location will be indicated for all key personnel. A description of management and key personnel relocating for the Project and how they will interact with local staff may be noted.
  • Continuity of Project Staff: The Offeror should provide a narrative to address how it will maintain the quality and depth of staff resources throughout the duration of the contract. MSDGC shall have approval authority regarding replacement of any key staff.

OVERALL PROJECT APPROACH (ATTACHMENT 3 TO RFQ)

Instructions for Narrative: Provide no more than 15 pages of narrative single-spaced in 12-point font on 8 ½ x 11 sheets with 1-inch margins.

General Information:The Offeror should describe its overall project approach to maintain scope, schedule, and cost control, manage project risk, perform quality control and quality assurance of deliverables, resolve resource constraints, manage change, and interact effectively with MSDGC staff from multiple divisions and its stakeholders. Provide a statement of project understanding and description of an approach for successfully implementing the Project. The submittal will be evaluated for scoring under this category based on the following information:

  • Work Plan: A management/technical approach regarding utilization of specific methodologies and techniques to perform the scope of work outlined in the RFQ should be provided. Regardless of whether the MSDGC awards a contract, the information submitted pursuant to this RFQ, including the data, information, concepts and ideas contained therein, shall be “works made for hire” and MSDGC shall have the right to use the information for the purposes of MSDGC, in any manner or combination it so elects, without notice to or the consent of the Respondents. Such “purposes” do not include the publication, distribution, or sale of such information to third parties not employed by or under contract to MSDGC, except as required by Ohio public records law or in connection with requests to perform construction work or design, or consulting services on behalf of MSDGC. Notwithstanding the foregoing, the City agrees that any use of such information without the Respondent's verification or adaptation for the specific purpose intended shall be at the sole risk of MSDGC.
  • Preliminary Master Schedule: Present a preliminary master project schedule that identifies major activities. The schedule shall clearly explain logic patterns and interdependencies and demonstrate understanding of sequence of events, tasks, phases, and deliverables. The schedule should clearly show key workshops and meetings necessary with MSDGC and other stakeholders including regulatory agencies; QA/QC activities, and technical reviews. For this submittal, assume a two and half week review of major deliverables by MSDGC after the hardcopy submittal has been received and logged by MSDGC Document Control. Present a reasonable length of time for written comment resolution. MSDGC will require a one week period for acceptance of written comment resolutions after receipt by MSDGC Document Control.
  • Cost and Schedule Controls: A narrative on how the Offeror will use schedule and cost analytical tools to maintain project schedule and budgets using earned value techniques and for the multiple tasks and concurrent related project activities.
  • Risk Management: A description of what the Offeror believes are the major project risks with the project as presented in this RFQ and ideas or suggestions for possible mitigation approaches.
  • Key Decision Points: Provide a narrative or flow diagram demonstrating the Offeror understands the key decision points regarding the Project and the appropriate timing of actions required for successful implementation of the GMWWTP Project. Discuss the relationship between key decision points and the success of the Project.
  • Quality Control: The Offeror shall provide a summary of their internal quality control procedures to describe how quality will be incorporated and maintained throughout the project during planning, design, and construction.
