All packages containing Responses must clearly state the RFQ No. (located on upper right hand corner of this page) on each outer mailing envelope or packing box.
Responses should be submitted in a tabbed and labeled binder, not permanently bound Transmittal letter and all required forms should be placed in First Tab of Binder.
The Original Response and each required copy must be clearly marked with the RFQ No. on the outer binder or cover page. The Original Response must be labeled “Original” and must contain the original, ink-signed transmittal letter, in addition to your Response.
All extra sets must be photocopies of the ENTIRE original Response submitted, including the transmittal letter.
Do NOT return this RFQ document or the sample “Model Contract.” Keep them for your reference.

A.Due Date for Proposals
Proposals must be submitted to the attention of the Suffolk County Office for the Aging staff member listed on page one of this Addendum, by the date set forth on page one of this Addendum.

In the interest of fairness to all participants, no extensions or exceptions will be permitted, unless issued as an Addendum to this RFQ and applicable to all Proposers.
B.Alternative Proposals
The Proposer may submit alternative Proposals, if there is more than one viable approach to the project.

The written technical questions received by the due date are answered here.
Question 1:Will the agency be allowed to include admin costs in their proposed budget with the RFQ submission?
Answer 1:Yes, You may include Administrative costs in your proposed budget.
Question 1A:Is the entire amount to be awarded $110,000 or will there be multiple awards of up to $110,000 with the plan to cover the entire County with these services?
Answer 1A:$110,000 is awarded once and may be split amongst multiple agencies.
Question 1B:If multiple awards, may an agency submit for more than one area of service?
Answer 1B:There is only a total of $110,000 for these services, one award.
Question 1C:With submissions, are you looking for the number of people to be served and a unit cost? For example, one could serve more people in a support group vs one to one counseling so the “unit” of service may be different. Or is a “unit” defined by a per hour of service?
Answer 1C:Cost Responses for counseling, support groups and training should be presented as a line item budget for the contract term. The Cost response should include all the costs involved in the design, administration, etc. of the project. One (1) unit of service is equal to each participant of a group or individual session.
Question 1D:Finally, when would you anticipate posting the answers to all technical questions and how will you respond? Via email from those who have expressed an interest?
Answer 1D:All technical questions are answered via email and online to all prospective responders. Answers are posted within a week of the technical question due date.
