* Please specify initial Lead-times and then on-going lead-times.
* Warranty/Damage issues will be handled by the supplier.
* Freight Terms must be Destination Freight Allowed (DESTFA) so all freight issues will be handled by the supplier.
* In order to prevent shipping losses, the Kits must be shipped in master cartons. Bidders must specify quantities for each master carton. I.e. How many Kits are included in each master carton? And, how many master cartons are on a pallet?
* Each supplier must provide a sample Kit for review which should include each of the items listed as a part of the kit. Note: if material is not available in time to provide samples, the bidder must provide specification sheets for each item.
* The purpose of this Bid event is to set up a Purchase Contract (PK) that will allow for subsequent PO Releases as needed. The PK will be for one year with the possibility for extending for additional years (to a max of 3 years). Pricing shall be firm for the first year and may be reviewed for the subsequent years based on market trends and pricing.
* Please provide any small/diverse supplier certifications as may apply to your company.
* Target date for initial delivery is January 1, 2018.
* Bid pricing should include:
* Overall price of the kit with specific breakdown for each line item listed in the "Kit Contents" section below; as well any other costs associated with each kit.
* One-time charges, if applicable and not included in above pricing.

1ea - Full color box with logo and graphics on 5 sides (Note: graphics will be provided to winning bidder)
3ea - LED bulbs (see below specifications)
2ea - '5-minute' Shower Timers with one-color imprinted logo
1ea - LED Night Light with Sensor and with one-color imprinted logo
1ea - Thermostat Temperature Gauge with one-color imprinted logo
Note: MPC will provide inserts (documents/brochures/etc) for the supplier to add to each kit.
