West Town Surgery is a single handed practice with support from regular locums. Dr U Khan is the lead General Practitioner.

West Town Surgery was established in 1987 by Dr Muraleedharan and moved to its present location in 1998. The current site is a Grade 2 Listed building with facilities for the disabled including access, internal facilities and parking. The practice is registered to dispense to a small number of patients. West Town Surgery is contracted to NHS England under a PMS contract and is a member of the North Lincolnshire Clinical Commissioning Group. Our Practice area covers Barton upon Humber, and the surrounding villages of Barrow upon Humber, Barrow Haven, South Ferriby and Burnham.

Our 3059* service users are the whole population, all ages with all medical conditions and disabilities. The age breakdown of our whole population of 3059* is as follows:

Under the Health and Social Care Act 2008 (The Care Quality Commission (Registration) Regulations 2008 Part 4), the registering body (West Town Surgery) is required to provide to the Care Quality Commission a statement of purpose.

Aims and Objectives for Delivering Each of the Above Regulated Activities

West Town Surgery will aim to ensure high quality, safe and effective services and environment and will aim to provide monitored, audited and continually improving healthcare services. We will aim to provide healthcare which is available to a whole population and to create a partnership between the patient and health professional which ensures mutual respect, holistic care and continuous learning and training. We will ensure the provision of accessible healthcare which is proactive to healthcare changes, efficiency and innovation and development.

Services Provided By West Town Surgery

West Town Surgery provides routine medical checks and general medical services including NHS relevant prescriptions and medications. Private prescriptions can also be issued and medication dispensed when appropriate.

Immunisations- We provide childhood immunisations and immunisations in relation to foreign travel.

Health Protection- Health protection services offered includes preventative immunisations e.g. Influenza immunisation and the NHS Health Check.

Lifestyle Management, Weight loss, Exercise and Smoking Cessation Advice- These services are available to and promoted to all patients.

Medicals- Under the non-NHS funded services our GPs are able to carry out medical examinations e.g. in relations to employment, and complete medical reports.

Respiratory Clinic- West Town Surgery provides a specialist clinic to support those with respiratory diseases such as Asthma and COPD including facilities for spirometery and lung function testing.

Diabetic Clinic- West Town Surgery operates a specialist diabetic clinic lead by the Practice Nurse at least once per month when patients will be directed to these specific appointments.

Family Planning and Contraceptive Services- West Town Surgery provides family planning and contraceptive advice to all patients upon request. Oral contraception is managed by the Doctors. Contraceptive implant and Mirena Coil fitting are offered at the practice and carried out by Dr U Khan.

Phlebotomy- West Town Surgery offers dedicated Phlebotomy clinics on a Tuesday afternoon from 3pm to 5pm and Wednesday & Thursday mornings from 9am to 12noon. For patients unable to attend during this time the Practice Nurse is happy to perform blood tests in her own clinics or make arrangements for the patient to attend the local pathology lab.

Minor Surgery- The surgery is able to offer a number of minor surgeries to patients. This includes steroid injections and procedures for dermatology related concerns. These procedures are managed by Dr U Khan.

Ear Syringing- West Town Surgery provides facilities for ear syringing, a service managed by our Practice Nurse.

Well Person Checks- Our practice nurse is available to perform well person checks. The practice also operates the NHS Health Check service for patients that meet the eligibility criteria.

Wound Dressings- Our Practice Nurse is trained in all aspects of wound care and all of our consulting rooms are equipped to manage patients with wound issues.

ECG Monitoring- The Practice Nurse and Health Care Assistant are able to perform ECG’s for patients in surgery if requested by a clinician.

Ambulatory Blood Pressure Monitoring- West Town Surgery has provision for Ambulatory Blood Pressure Monitoring.

Cervical Screening- Our Practice Nurse is qualified to carry out cervical screening tests in the form of cervical smears.

Psychology Therapy (Workers) - The practice allows POEISIS workers to host weekly clinics within the surgery to allow easy access to psychological therapies for patients.

Carers Support- West Town Surgery has a close relationship with the local carers support service allowing them to promote carers support in the surgery.

Community Services- The practice also has Physiotherapy, Chronic Wound Management, Podiatry and Dieticians attend the surgery on a regular basis. Patients are often referred to these services.

NHS CCG Medicines Management- Once a week we have a Pharmacist attend the surgery from the CCG Pharmaceutical Advisors.

We treat both patients and illnesses. This means that we are equally interested in the physical, physiological and social aspects of your individual care.

Continuity of Care

Building and maintaining strong relationships between Doctors, Health Professionals and patients is essential to the way we work. This is especially so in the management of on-going problems or long-term illness. In these circumstances we would encourage you to continue seeing the same Health Professional and where ever possible we will facilitate this through our appointments system. However, if you have a new problem, and the doctor or nurse you normally see is not available, then we would encourage you to see any of the Doctors or Nurses at the practice.

Learning and Training

West Town Surgery is a teaching practice, and year 3 Hull York Medical School students spend time in the practice on a regular basis as part of their rotation. They are fully supervised by Dr Khan to gain experience as part of their course. As part of being a teaching practice, West Town Surgery is annually inspected by Hull York Medical School to maintain standards and be able to continue to teach.

We believe in ‘lifelong’ learning and all the health professionals and administrative staff here undergo an annual appraisal where the goals of the individual, team and practice are discussed and agreement reached on the way forward. Regular reviews act as a way of reinforcing effective performance, highlight areas for improvement and recognise developing strengths.

We also recognise the benefit of supported learning for our patients and families in enhancing their ability to manage and deal with both ‘self-limiting’ and long term illness.
