Review Request Pro-Forma

Review is the formal means by which a student may call into question the decision of a Board of Examiners.

If you wish to request a Review of a Board of Examiners’ decision under section B1.2 of the University General Regulations 2017/18, you should complete this form in full and submit it to the Academic Appeals Officer, Secretariat, 3rd Floor, Minerva Building, University of Lincoln, Brayford Pool, Lincoln, LN6 7TS, within 10 working days of being formally notified of the decision of the Board of Examiners.

Before completing this form:

You are encouraged to discuss the matter with your Academic Tutor, Module Co-ordinator, Lecturers, Programme Leader, Students’ Union officers or Student Services staff as appropriate in an attempt to resolve the issue informally.

If you wish to proceed to lodge a formal Review request, please read section B of the University General Regulations 2017/18, which sets out the procedures involved. (a)  Please state precisely the decision(s) of the Board of Examiners against which the Review is requested (e.g. Failure, Resit etc.)

Please specify which piece(s) of work/assessment/exam/modules are the subject of the dispute. Please note only the piece(s) of work/assessment/ exam/modules cited on this form will be considered; additional piece(s) of work/assessment/modules cannot be introduced at any later stage of the process.

(b)  Have you attempted to resolve your concerns informally and if so, please provide details (e.g. whom you approached and why the issue has not been resolved to your satisfaction.

(c)  The Review stage of the Appeals process is a pre-requisite for a formal Appeal; it offers students the opportunity to seek assurance that all material circumstances were taken into account by a Board of Examiners when it arrived at its decision. Please note: you must provide ALL material and evidence that you wish to be considered in support of your Review at the time that the Review request is made. Matters of contention not raised at the Review stage cannot be introduced at the Appeal stage should you remain dissatisfied following the outcome of the Review.

Please state as concisely as possible the reasons for requesting a Review and why these would have been material to the decision of the Board of Examiners and attach appropriate documentary evidence. If your reasons for requesting a Review relate to extenuating circumstances, please see section (d) below.

Please note: complaints against the standard of tutoring etc. must be made before the results of the Board of Examiners and cannot be cited in a Review request or subsequent Appeal. Complaints of this nature, if not resolved informally, must be made through the Student Complaints Procedure detailed in Part E of the University General Regulations 2017/18.

(d)  If the reasons for requesting a Review are extenuating circumstances, have they been considered by the University Extenuating Circumstances Panel? YES / NO (delete as applicable)

If they have not been considered by the Extenuating Circumstances Panel (ECP), please explain why you did not raise them at the time (please refer to University General Regulation B.2.2 (a) and B.2.2 (i) for further information). Students who submit extenuating circumstances as part of their Review Request will have their validity decided by the ECP before the Review Request is forwarded to the Chair of the Board of Examiners.

Please note: if a student, without good cause, fails to inform the ECP and hence the Board of Examiners in advance of its meeting about any extenuating circumstances that may have affected performance in assessment, the Board of Examiners, or any subsequent process of Academic Review and Appeal, has the authority to reject any such request to review the decision on those grounds.

Please provide as much detail as possible regarding the extenuating circumstances you wish to raise, including detail as to how the circumstances impacted upon your ability to study. You must provide independent, corroborating evidence (e.g. medical certificate) of any extenuating circumstances you raise.

(e)  Please state, as concisely as possible, why you believe that had the Board of Examiners been aware of the circumstances set out in (c) and / or (d) it would have made a different decision to the one made.

(f)  Without prejudice to the outcome of the Review, please state the resolution that you are seeking.
